Summary of Statement of Work: Durable Medical Equipment (DME) Kits / PUSH Kits

This is a Statement of Work (SOW) issued by the Federal Emergency Management Agency (FEMA) under the Department of Homeland Security for the supply of Durable Medical Equipment (DME) Push Kits and full kits to support disaster relief operations. The DME kits are designed to provide medical equipment support for individuals requiring durable medical equipment in emergency shelters, addressing a gap where shelter providers lack resources to support functional needs. Kits support a shelter population of 200 persons and contain 47 line items for PUSH Kits and 51 line items for full kits. Each full DME Kit includes specialized medical equipment such as bariatric cots, wheelchairs, walkers, commodes, transfer lifts, shower chairs, assistive listening devices, adapted eating devices, pediatric wheelchairs, and refrigeration units. FEMA reserves the right to modify kit contents based on emergency-specific needs and may order full kits or individual commodities on an à la carte basis.

Contractors must meet demanding delivery requirements under both emergency and steady-state conditions. During emergency response, contractors must deliver up to 702 complete DME kits within 72 hours to any CONUS or OCONUS location specified by FEMA. During steady-state operations, contractors must deliver a minimum of two kits to any CONUS Distribution Center within 10 days or to OCONUS locations within three weeks. Deliveries must be made to specified FEMA Distribution Centers located in Atlanta, Georgia; Greencastle, Pennsylvania; Fort Worth, Texas; Tracy, California (CONUS) and Guam, Hawaii, and Puerto Rico (OCONUS). All DME kits must be packaged in standard commercial conex containers on industry-standard pallets (48"L x 40"W, maximum height 55") with minimum 10-layer stretch wrap covering all sides except top and bottom. Each pallet requires labeling with packing slips, manufacturer information, net quantities in both metric and imperial units, and special wrapping specifications for the bariatric wheel ramp (DM-024).

DEPARTMENT OF HOMELAND SECURITY (DHS)

Federal Emergency Management Agency (FEMA)

STATEMENT OF WORK

Durable Medical Equipment (DME) Kits / PUSH Kits (Steady State / Emergency Declaration)

1.0 Project Title

Durable Medical Equipment Kit / PUSH Kit

2.0 Background

There is a longstanding gap in shelter support for individuals requiring Durable Medical Equipment (DME) during disasters. Many arrive at shelters without their equipment or caregivers, and are often denied admission or redirected to hospitals, which may reserve beds for critical cases only. Most shelter providers lack resources to support functional needs, and there is currently no federal contracting mechanism to provide DME support in shelters.

To address this, FEMA is establishing a process to provide DME Push Kits to shelter providers upon request by a State, pursuant to Section 403(b) of the Robert T. Stafford Disaster Relief and Emergency Assistance Act (Public Law 93-288, as amended).

3.0 Scope

Contractors shall supply DME Push Kits for FEMA purchase as needed, per requirements from FEMA Headquarters Distribution Center Supply Chain / Emergency Assistance Unit. FEMA will warehouse these kits, which contain 47-line items for PUSH Kits and 51 for full kits. Each kit supports a shelter population of 200 persons. Delivery orders may require full kits or individual commodities on an ala carte basis.
