About this file
This document is a Disqualification Disclosure form required by California Public Contract Code §10162, specifically associated with the One Stop Operator Services for the Foothill Workforce Development Board's Workforce Innovation and Opportunity Act (WIOA) 2023 procurement. The form requires prospective bidders to disclose, under penalty of perjury, whether the bidder, any officer, or any employee with a proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.
The disclosure form is a standard compliance document that allows the contracting agency to assess the bidder's past performance and legal standing. By requiring bidders to check "Yes" or "No" and potentially provide additional documentation if there are prior disqualifications, the form helps ensure transparency and accountability in the government contracting process. This specific document is part of the procurement process for a federally funded workforce development service, with funding sourced from the U.S. Department of Labor under the Workforce Innovation and Opportunity Act (WIOA) Title I.
