About this file

This document is a Disqualification Disclosure form required by California Public Contract Code (PCC) §10162, specifically for the City of Pasadena's 457 Deferred Compensation Plan Services contract opportunity. The form requires prospective bidders to disclose under penalty of perjury whether the bidder, any officer, or employee with proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The disclosure form provides a simple yes/no checkbox mechanism for bidders to self-report any previous disqualifications, with an option to explain and attach documentation if the answer is affirmative. This requirement is part of the City of Pasadena's due diligence process in selecting a consulting firm to manage its 457 Deferred Compensation Plan, ensuring transparency and regulatory compliance in the procurement process for financial services.
