About this file
The document is a Disqualification Disclosure form from California, specifically aligned with Public Contract Code §10162, requiring prospective bidders for the Pasadena Fire and Police Retirement System's Investment Consultant Services to disclose any previous disqualifications from government project bidding. The form requires bidders to indicate whether they, their officers, or employees with proprietary interest have ever been disqualified, removed, or prevented from bidding on or completing federal, state, or local government projects due to law or safety regulation violations.
The disclosure form is a mandatory component of the bidding process for the Investment Consultant Services opportunity, which seeks a firm to provide general investment consulting services to the Pasadena Fire & Police Retirement System. By requiring this disclosure under penalty of perjury, the City of Pasadena aims to ensure transparency and accountability from potential service providers, with bidders required to check either "Yes" or "No" and provide explanatory documentation if they have experienced any past disqualifications that might impact their ability to successfully complete the government contract.
