About this file
The document is a Disqualification Disclosure form in accordance with California Public Contract Code §10162, specifically related to the City of Pasadena's Non-Exclusive Franchise Towing Agreement. The form requires prospective bidders to disclose whether the bidder, any officer, or any employee with a proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.
The form is a standard legal declaration where bidders must check either "Yes" or "No" to indicate their compliance history, with an option to explain and attach documentation if they have been previously disqualified. This disclosure is part of the procurement process for the City of Pasadena's towing services contract, which seeks non-exclusive franchises for vehicle towing and impound services to be managed by the Pasadena Police Department and Department of Transportation.
