About this file
This document is a Disqualification Disclosure form in compliance with California Public Contract Code (PCC) §10162, specifically related to the Pasadena Public Library's Library Collection Relocation Services contract opportunity. The form requires prospective bidders to disclose under penalty of perjury whether the bidder, any officer, or employee with proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to violations of law or safety regulations.
The form provides a simple yes/no checkbox mechanism for bidders to self-report any past disqualifications, with an option to attach explanatory documentation if the answer is affirmative. This disclosure is part of the bidding process for the library collection relocation project, which seeks professional services for transferring the library's collection during a central library retrofitting process. The form represents a standard procurement transparency mechanism designed to ensure bidder accountability and compliance with government contracting standards.
