About this file

The document is a Disqualification Disclosure form in compliance with California Public Contract Code §10162, prepared for the Pasadena Water and Power (PWP) Marketing, Design and Communications Services contract. The form requires bidders to disclose whether the bidder, any officer, or employee with proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The form is a standard legal disclosure document that allows bidders to transparently report any past project disqualifications under penalty of perjury. By requiring this disclosure, PWP can assess the bidder's past performance and compliance history as part of the evaluation process for the marketing and communications services contract related to their 100% carbon-free by 2030 initiative. Bidders must check either "Yes" or "No" to the disqualification question, with an option to provide explanatory documentation if the answer is affirmative.
