About this file

The document is a Disqualification Disclosure form in accordance with California Public Contract Code (PCC) §10162, specifically related to the City of Pasadena's procurement process for an Independent Registered Municipal Advisor (IRMA). The form requires prospective bidders to disclose whether the bidder, any officer, or employee with proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The form is a standard compliance document that asks bidders to check either "Yes" or "No" to the disqualification question, with an option to explain circumstances and attach supporting documentation if the answer is affirmative. This disclosure is part of the City of Pasadena's due diligence process for selecting a qualified IRMA professional firm to provide independent financial advisory services, as outlined in the related contract opportunity description involving debt issuance, financial structuring, and regulatory compliance with SEC and Municipal Securities Rule Rulemaking Board requirements.
