About this file
The document is a Disqualification Disclosure form required by California Public Contract Code (PCC) §10162, specifically for the City of Pasadena's Economic Development Strategic Planning Services procurement. The form requires prospective bidders to disclose under penalty of perjury whether the bidder, any officer, or employee with proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to violations of law or safety regulations.
The disclosure form is a standard compliance document that allows the City of Pasadena to assess the bidder's past performance and legal standing before awarding the contract for economic development strategic planning services. Bidders must check either "Yes" or "No" to the disqualification question, with the option to attach explanatory documentation if they have previously been disqualified. This form is part of the city's due diligence process to ensure contractor integrity and minimize risk when selecting a consultant for their five-year strategic planning initiative.
