About this file
The document is a Disqualification Disclosure form in accordance with California Public Contract Code (PCC) §10162, designed for prospective bidders in a state and local contract opportunity. The form requires bidders to disclose under penalty of perjury whether the bidder, any officer, or any employee with proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.
The form provides a simple yes/no checkbox mechanism for bidders to self-report any historical disqualifications, with an option to explain circumstances and attach supporting documentation if the answer is affirmative. While the document is part of a standard procurement process, the specific contract details are not elaborated within this form, which serves primarily as a transparency and accountability mechanism for government contracting.
