About this file

The document is a Disqualification Disclosure form required by California Public Contract Code §10162, specifically for the City of Pasadena's Parks, Recreation and Community Services Department's Recreation Contract Instructors (Dance) opportunity. The form is a standard compliance document that mandates bidders to truthfully respond "Yes" or "No" to the disqualification question, with an option to attach explanatory documentation if the answer is affirmative. This disclosure is designed to ensure transparency and integrity in the bidding process for the City of Pasadena's recreation contract instructor positions, helping to maintain high standards of professional conduct among potential service providers.
