Offerors shall provide their references with the letter and questionnaire on the following pages. References are to complete the questionnaire and submit it directly to the Contracting Officer/Contract Specialist before the due date of the Request for Quotes.

Questionnaires submitted by the offeror will not be considered.

SUBJECT: Request for Past Performance Information Solicitation #DE-SOL-0009686

The Western Area Power Administration will be evaluating quotes received in response to the subject solicitation for contractor(s) to provide Bareground Weed Control Services. One of the integral considerations in the evaluation is the verification of the offeror’s past performance on contracts, which reflect the offeror’s ability to perform on the proposed effort. We depend on information received from agencies/firms such as yours, which have had first-hand experience with the offeror, and for the evaluation of their performance.

Our areas of interest in the offeror are summarized in the enclosed questionnaire.

You are urged to submit your comments on other similar contracts your entity has/had with the offeror. Any additional information that you think may benefit the Government in our evaluation of the offeror's quote is appreciated. Please note that we may contact the offeror on problem areas or concerns identified, to determine whether corrective actions have been taken or are in process. The offeror is required by our solicitation to provide points of contact for each reference provided to the Government.

The name of the individual completing the questionnaire will not be disclosed to the offeror.

Due to time constraints it is imperative that the questionnaire be completed and returned no later than May 10, 2016, to Eileen Ray, Contracting Officer, via e-mail at eray@wapa.gov.

Part III – Questionnaire

When responding to the following questions/statements below, choose the number on a scale of 1 to 5 which most accurately describes the Contractor’s performance.

The numbers correspond with the following: 1 = Unsatisfactory, 2 = Marginal, 3 = Satisfactory, 4 =Very Good, 5 = Exceptional

1. Contractor effectively manages the effort required by the contract, develops realistic schedules, met established schedules, and realigned schedules to meet customer needs when required.

2. Contractor provides/hires experienced supervisors and qualified personnel with technical and administrative abilities needed to meet contract requirements. Demonstrated ability to replace personnel, if necessary without delays.

3. Contractor displays initiative to solve problems. Suggests approaches to problems and performs as a “team” member.

4. Contractor requests additional or clarification information at appropriate times.

5. Contractor is responsive and provides timely resolution for correction of problems.

6. Contractor provides accurate invoices that reflect actual work completed.

7. Contractor is knowledgeable and experienced with different kinds of herbicides and equipment used to control weeds.

8. Contractor was able to complete the weed control in an efficient manner with few if any reapplications.

Unsatisfactory Contractor. We would not want to contract with this firm again under any circumstances.

Marginal Contractor. Numerous problems developed that were a result of their lack of cooperation and failure to perform work as required.

Satisfactory Contractor who met the minimum requirements of the contract.

Performance deficiencies, if observed, improved when identified by the Government.

Rework/warranty issues, if applicable, were performed; however, an aggressive inspection program was required to ensure compliance.

Very good Contractor with whom we would not hesitate to enter into another contract.

Problems encountered were minor and solutions were found with little difficulty.

Exceptional Contractor in every respect. Problems were solved in a spirit of teamwork. Quality work, timely service, and complete documentation were routinely achieved. We would pay premium prices to contract with them again.
