The Agency hereby deletes Appendix IX, in its entirety and replaces it with the following:

APPENDIX IX Cost Sheet

The following criteria will be used to evaluate and rank the Cost Proposals.

Price analysis will be based on a comparison of the Cost Proposals submitted. The proposal with the lowest acceptable price will receive the maximum points. Proposals with higher prices will receive a fraction of the maximum points.

Maximum Points Available

Cost Proposal for Services: Max 50 points Cost Proposal for Program Administration: Max 125 points Cost Proposal for Community-Based Partnerships & Outreach: Max 50 points

Total Maximum Points: 225 points

How Points are Awarded

Total Cost for All Categories:

For each vendor, the total cost will be calculated by adding together the costs for the three categories:

Cost Proposal for Services + Cost Proposal for Program Administration + Cost Proposal for Community-Based Partnerships & Outreach

Formula for Awarded Points:

The total cost for each vendor will be divided by the lowest total cost (from all vendors) and then multiplied by the total available points (225 points). This is calculated as follows:

Awarded Points = (Lowest Total Price for All Categories / Respondent's Total Price for All Categories) x 225 points

Example:

• Vendor A has the lowest total price of $350,000. Since Vendor A has the lowest price, they receive the maximum 225 points.

• Vendor B has a total price of $375,000.

Using the formula:

(Lowest Total Price / Vendor B's Total Price) x 225 points ($350,000 / $375,000) x 225 = 210.0 points

Awarding the Contract

The contract will be awarded to the responsible and responsive vendor whose proposal is determined to be the most advantageous to the state, considering both price and the other criteria outlined in this RFP.
