SUBJECT: Request for Past Performance Evaluation

________________________ is currently participating as an offeror in response to the Government’s Request for Quote Number FA5000-22-Q-0022 for the Demolition of Building 7250, Firm-Fixed contract at Joint Base Elmendorf/Richardson, Alaska.

The government will be selecting contractors for this project based on selection factors. One of the selection factors to be rated is contractor past performance. It shall be a key factor in the government’s final decision. To assist the government in its assessment of contractor past performance, we are required to identify previous projects and clients who are familiar with our past work.

As one of our prior/current customers for past contracting work on project ___________________, we are requesting that you complete the attached Past Performance Questionnaire regarding our past performance. This letter also authorizes you to discuss our past performance with the government and to release information regarding our past performance to the government.

Your prompt attention in completing the attached Past Performance Questionnaire will assure we receive full consideration for the contracts to be awarded. Please return the completed form to the government representative POC identified below, to arrive no later than the proposal due date.

Satisfactory Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.

Marginal Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being evaluated reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor’s proposed actions appear only marginally effective or were not fully implemented.

3. Did your service contract include site restoration?

Performance meets contractual requirements and exceeds some to the government‘s benefit. The contractual performance element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were highly effective.

Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
