About this file
This is a Government-Wide Debarment & Suspension Certification form required for the City of Rock Hill, South Carolina's procurement of a diesel pump for the Manchester Creek Wastewater Treatment Plant. The certification document serves as a compliance requirement for contractors bidding on this state and local contract opportunity. Contractors must certify that their principals are not debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from covered transactions by any Federal department or agency. Additionally, contractors must affirm that they and their principals have not been convicted of or had civil judgments rendered against them for fraud, criminal offenses, antitrust violations, embezzlement, theft, forgery, bribery, falsification of records, false statements, or receiving stolen property within the three-year period preceding the bid submission.
The certification further requires contractors to confirm they are not presently indicted or criminally or civilly charged by any governmental entity with commission of the enumerated offenses, and that they have not had one or more public transactions terminated for cause or default within the three-year period preceding the solicitation. Contractors unable to certify to any statements in the certification must attach a written explanation to the document. The authorized official of the contractor must sign and date the certification, affirming the truthfulness and accuracy of the submitted statements, with understanding that the provisions of 31 U.S.C. Sections 3801 et seq. apply to the certification.
