About this file
This is a Darfur Contracting Act Certification form issued by the California Department of General Services (DGS) Procurement Division that establishes compliance requirements for companies bidding on State of California contracts. The form applies to any company with business activities or operations outside the United States within the previous three years. Bidders must certify either that they are not a scrutinized company under Public Contract Code Section 10476, or they must provide written permission from the DGS Director authorizing them to submit a proposal as a scrutinized company. Companies without international business activities within the past three years are exempt from completing this certification. The form requires the authorized company representative to sign and certify under penalty of perjury their eligibility status, providing the company name, Federal ID Number, and signature with printed name and title.
The certification applies specifically to the IFB S25-33223 Vending Machine Revenue Contract for one location in Santa Rosa, California, issued by the Department of Rehabilitation. This document is a mandatory compliance requirement that must be submitted as part of the required attachment checklist for bid submission. Bidders are responsible for understanding all requirements and requesting clarification prior to submission through the Contract Analyst, Nathaniel.Hayes@dor.ca.gov. The solicitation references the standard General Terms and Conditions (GTC 02/2025) and Contractor Certification Clauses (CCC 4/2017) available through the DGS Office of Legal Services, and includes a Disabled Veterans Business Enterprise (DVBE) Incentive component. All questions and answers will be posted on the Cal eProcure website, and bidders are advised that only written official addenda are binding upon the State.
