DANCE INSTRUCTOR

Performance Work Statement

1. General: The contractor shall provide non-personal services in dance instruction consisting of labor, transportation, supplies and material in strict compliance with the terms and conditions herein.

2. Contractor Qualifications: Contractor must have a minimum 2 years of experience in instruction of dance at an established facility.

3. Type of class: 45 minutes of dance classes for youth 2-3 years of age, creative movement class, youth ages 4-8, a combination of ballet, tap and jazz.

4. Fee:$50 per month

5. Course Schedule:Hours and dates to be determined between instructor and Program Coordinator. Classes will be held from September until June. An end date for summer will be set by the POC and contractor. Summer classes can be offered and will be determined by the POC and contractor.

6. Location: JB MDL, NJ

7. Authorized Class Sizes: Minimum: 5 Maximum: 12

If a course does not meet the minimum amount of students, the instructor and Program Coordinator (POC) will discuss a registration deadline. Deadline will be set at the discretion of the POC. If the course does not meet the minimum requirement by registration deadline, the course will be suspended until class has met the minimum amount of students and the contractor will receive no payment for the suspended course during that time.

8. Class Cancellation:The classes can be postponed due to inclement weather, natural events including but not limited to, snow storms, hurricanes, water line breakage, power outages, etc. or illness of instructor. If there is base closure due to inclement weather or other unforeseen events, classes under this contract will be cancelled. The contractor will be responsible for notifying students in the event of a class being cancelled. The government reserves the right to reschedule class to allow for the maximum number of students to attend.Make up days will be offered in the event of a base closure or natural event.
