About this file

This is a Corporate Certificate template form required for lease agreements with the General Services Administration. The form must be completed by someone other than the individual who signed the lease document to certify their authority to sign on behalf of the lessor corporation. It includes fields for the certifying official's title, the name of the person who signed the lease, their position in the corporation, and confirmation that the document was properly authorized within the scope of corporate powers.

The form includes a Privacy Act notice stating it is used under 40 USCA Section 49(h)(1) to verify property ownership and signing authority. While disclosure is voluntary, failure to provide this certification will prevent the lease from being finalized. The document requires both a date and signature from the certifying official.
