Statement of Work for Services (SOW)

General Information:

Title of Project:

1) One year Silver Service Agreement for Illumina MiSeq NG Sequencer

2) One year Bronze Service Agreement for NextSeq Sequencer

Statement of Need and Purpose:

NCATS is anticipating greater use of Illumina Sequencers in the coming year and would like to place the instruments under a service plan as insurance against potential service issues with the instrument. The agreement will include a PM for the MiSeq instrument so that all systems are up to original factory settings.

The agreement will also cover any Software updates and phone support for any potential issues that may arise

Background Information and Objective:

The Gerhold Laboratory at NCATS is developing an assay that will allow higher throughput of gene expression studies for Toxicity/ADME projects. This assay will highly leverage the use of Next Generation sequencers such as the Illumina MiSeq and NextSeq that are presently in the laboratory. The laboratory has experienced a high number of service related issues with instruments in the lab and so a service plan would alleviate some of the technical issues that may develop. The service plan would also allow a PM to be performed on theMiSeq instrument and would also allow us to contact the Illumina customer care center directly for troubleshooting of any assay related issues (i.e. reagent choice, data handling, etc)

Period of Performance:

The Base Period of Performance is 12 months with four (4) successive one-year options for a total five-year contract.

Scope of Work:

The agreement will cover all parts, labor and travel, reagent replacement for HW failures, technical support software and hardware updates, on site applications support and remote monitoring. The agreement will also provide phone support for the instrument.

2) The Illumina Bronze Service agreement for the NextSeq Instrument will agreement will cover all parts, labor and travel, reagent replacement for HW failures, technical support, software and hardware updates, on site applications support and remote monitoring. The agreement will also provide phone support for the instrument.

Level of Effort:

1) The Silver plan is the most popular full-service plan balancing performance, productivity, and cost to deliver confidence and peace of mind.

2) The Bronze service plan is the most cost effective plan for the NextSeq Instrument to deliver confidence and peace of mind.

Government Responsibilities:

NCATS will provide access to the Seqencing Instruments and will perform any measures to elimate the chance for contamination of any hazardous materials to the service technicians.
