About this file
The document is a Past Performance Questionnaire (D.8) designed to evaluate a contractor's performance across multiple dimensions. The questionnaire requires detailed assessment of a contractor's performance using a 1-6 rating scale across categories including quality of product/service, price control, timeliness/schedule, business relationships, and key personnel. Evaluation criteria cover specific aspects such as conformance to contract requirements, accuracy of reports, personnel appropriateness, management effectiveness, communication, responsiveness, and overall contract performance.
The questionnaire is to be returned by July 16, 2025, to Patricia Trusler at patricia.trusler@va.gov. It includes sections for respondent and evaluatee information, contract details, and performance ratings. The final section requires an overall performance rating (ranging from Exceptional to Unknown) and allows for comments, with a mandatory explanation required for any Unsatisfactory, Marginal, Very Good, or Exceptional ratings. The document also includes yes/no questions about contract termination, cure notices, and willingness to award another contract to the evaluated contractor.
