About this file

This document is a request form for extending per diem payments beyond 180 days for veterans residing in Health Care for Homeless Veterans (HCHV) contracted residential care facilities. The form must be completed by the contractor and submitted to the HCHV Coordinator/VA Liaison as soon as an extension is needed, with reasons for the request and the veteran's anticipated departure date. The VA liaison must confirm that clinical circumstances beyond the contractor's control prevent earlier placement, all efforts to place the veteran sooner have been exhausted, and continued residence would provide therapeutic benefits. If approved by the contracting officer representative, per diem payments may continue until a specified date for further review. The related federal contract opportunity is to provide low demand safe haven services at the Canandaigua VA Medical Center under solicitation number 36C24223R0004 by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 2.
