About this file
This Excel spreadsheet template is for calculating fuel surcharges for a wheelchair transportation services contract with the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 10. The contract period of performance is from 1 April 2023 to 31 March 2026, with options to extend for two additional one-year periods. The template provides for input of average fuel costs on a quarterly basis based on data from the U.S. Energy Information Administration for the Cleveland, Ohio area. Price per mile and costs for wear and tear are to be established at time of proposal and remain constant throughout the contract period. Proposed adjusted mileage rates incorporating fuel costs are also to be included.
