About this file
This document is a Single Source Justification form for a simplified acquisition procedure (SAP) under the Simplified Acquisition Threshold (SAT) for the Department of Veterans Affairs, VA Central Iowa Health Care System. The VA requires a temporary mobile PET/CT trailer from Siemens' Biograph series to replace their existing imaging system during a 2.5-3 month equipment replacement and site preparation period. The justification details specific technical requirements that limit the procurement to a Siemens Biograph mobile PET/CT, including hybrid PET/CT functionality, 32,000 LSO detectors, time-of-flight technology, ECG and respiratory gating, a 70cm bore size, patient weight capacity up to 450 lbs, and compatibility with the hospital's existing Siemens Sygno platform and workflows.
Market research conducted through VetCert revealed 382 registered SDVOSB/VOSB vendors under the relevant NAICS code, with 30 past awards for similar requirements and 18 total SDVOSB set-aside awards across five different SDVOSB vendors. The procurement will be competed among service-disabled veteran-owned small businesses, with the goal of obtaining a fair and reasonable price that offers the best value to the VA. The Contracting Officer, Joshua Imdacha, certified the justification on May 8, 2025, with an effective date of November 15, 2024.

2. Brief Description of Supplies/ Services required and the intended use/Estimated
Amount: The VA Central Iowa Health Care System (VACIHCS) in Des Moines, Iowa has a need for a temporary mobile PET/CT trailer while the Imaging Department’s current PET/CT system is being replaced. The rental duration is estimated to be 2.5-3 months during deinstallation of the existing unit, site-prep construction, and installation of the new unit. The mobile PET/CT trailer will be placed on the East side of Building 1. The required PET/CT is a Siemens PET/CT from the Biograph series. The contractor must provide transportation, 3 days of applications training, and hard drive removal. IGCE is
3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: VA Central Iowa is requesting a Siemens brand Mobile PET/CT system from the Siemens PET/CT Biograph series. Currently VACIH utilizes a Siemens Biograph mCT. Renting a system from the same series will ensure the same high level of patient care. The clinicians’ familiarity with the Siemens Biograph series’ software and interfaces will result in less training and downtime. Additionally, renting a Siemens Biograph unit will ensure system and software compatibility with the current system.
The mobile PET/CT must meet the following requirements that limit availability to the Siemens Biograph series:
To ensure operational consistency, the PET/CT must:
o A hybrid positron emission tomography (PET) and computed tomography (CT) into a single system, combining metabolic and anatomical imaging. This hybrid approach supports applications in oncology, cardiology, neurology, and radiation therapy planning, ensuring versatility across clinical indications.
o Utilize at least 32,000 Lutetium Oxyorthosilicate (LSO) detectors with a minimum crystal size of 3.2mm x 3.2mm to ensure consistent PET performance, enabling reliable lesion detection and quantification.
o Incorporate time-of-flight (TOF) technology of at least 178ps which enhances image contrast and signal-to-noise ratio by precisely measuring the time difference between detected photons.
o Support ECG-gated PET imaging for cardiac applications.
VHAPG Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Attachment 1: Single Source Justification for SAP under the SAT
Revision: 01 Effective Date: 11/15/2024 Page 2 of 3 Acquisition ID#36C263-25-AP-1629 o Support respiratory gating to reduce motion artifacts.
o Feature a bore size of at least 70cm to accommodate diverse patient sizes, including bariatric patients, and support radiation therapy positioning devices.
This standard bore size reduces patient positioning challenges across models, simplifying clinical routines.
o Support a patient weight limit of up to 450 lbs. enhancing accessibility for larger patients. This uniformity reduces the need for technologists to adapt protocols for patient handling across different models.
o Be equipped with a hot lab to receive and store the radioisotope and radioactive waste.
o At least 6 slice CT for image resolution and diagnostic accuracy.
o At least 2 ring PET for a larger axial field of view and higher sensitivity.
o Be equipped with an Intravenous (IV) contrast injector and warmer.
o Be equipped with Compact Disk-Recordable CD-R/DVD-R burning capabilities as a backup to network transfer of data.
o Be equipped with an Image Analysis Station for image registration quality assurance.
o Be equipped with a programmable injector with pressure monitor having the following capabilities (example unit would be the Medrad Stellant Single Syringe Injector):
Scan delay Multiphase option Protocol storing Pause hold functionality
To ensure data and software integration the mobile PET/CT must:
o Use Siemens’ Sygno platform for image processing, reconstruction, and quantitative analysis. This multi-modality tool supports deformable registration, dose visualization, and standardized reporting, ensuring consistent data handling and interpretation across models. Familiarity with this software reduces the learning curve for clinicians transitioning between Biograph systems.
o Incorporate dose reduction technology.
o Utilize TrueV technology which is a post-reconstruction image processing feature aimed at enhancing spatial resolution and image clarity.
o Up-to-date Microsoft (MS) patches and anti-virus software o Support consistent reporting and interoperability with hospital PACS (Picture
Archiving and Communication Systems).
Since our staff currently uses a Siemens Biograph system, a PET/CT trailer from the same model series will ensure seamless integration with existing workflows, software, and hardware. Since the service line is already trained on Siemens systems, onboarding a Siemens trailer would require less additional training, enabling quicker deployment and minimizing workflow disruptions.
