About this file
This is a contractor certification document required for the Therapeutic Apheresis Services solicitation (36C25226Q0268) at the Jesse Brown VAMC and Edward Hines Jr. VA Hospital under the Department of Veterans Affairs Veterans Health Administration.
The certification requires contractors and their subcontractors to comply with the Immigration and Nationality Act of 1952 and related laws enforced by Homeland Security, Immigration and Customs Enforcement, and the U.S. Department of Labor. Contractors must certify they will not knowingly employ or subcontract with illegal aliens or foreign nationals whose immigration status is in violation of U.S. admission requirements. Full compliance with E-Verify requirements under Executive Order 12989 and applicable Federal Acquisition Regulations is mandatory. Non-compliance may result in prohibition of non-compliant foreign nationals from the contractor's place of business servicing VA patient referrals and constitutes grounds for contract termination for breach. The document requires the contractor's signature, date, typed name and title, and company name, with notification that false certifications may result in prosecution under 18 U.S.C. Section 1001.
