About this file
This document is a Contract Amendment (Amendment No. 2) issued by the Arizona Department of Health Services (ADHS), Office of Procurement, for Contract No. CTR076383. The contract is between the Arizona State Hospital (ASH) and City Wide Pest Control, Inc., a domestic for-profit corporation based in Phoenix, Arizona. The amendment pertains to pest control services for the Arizona State Hospital, which includes the main hospital facility and the Arizona Community Protection and Treatment Center (ACPTC).
The contract requires City Wide Pest Control to provide comprehensive pest control services for the hospital's interior and exterior spaces, including thirteen separate units across three areas. Services include quarterly treatments for most areas and monthly treatments for kitchen spaces, targeting pests such as roaches, ants, silverfish, spiders, crickets, flying insects, scorpions, ticks, and rodents. The contractor must adhere to strict scheduling requirements, with specific time windows for servicing different hospital areas, and must comply with various regulatory standards including The Joint Commission, OSHA, NFPA, EPA, and CDC guidelines. Additional services may be provided on an as-needed basis, such as exterior power spraying, termite control, bird removal, and emergency treatments. The contractor is required to submit monthly invoices and reports detailing the date, time, and location of services performed.
