About this file

This document is Amendment No. 2 to Contract CTR072628, a Food Services Management contract involving the Arizona Department of Corrections, Rehabilitation & Reentry, located in Phoenix, Arizona. The amendment involves a contractor (partially redacted, appears to be an LLC) and modifies specific contractual terms related to compliance with department regulations.

The amendment specifically adjusts Section 19 of the Special Terms and Conditions, relating to compliance with department laws, rules, and regulations. While maintaining most existing contract requirements, the amendment explicitly exempts the contractor from adhering to Department Order 804, section 5.3.1.1, while still requiring compliance with other departmental rules and policies. The amendment preserves the overall scope of food services management for the corrections department, with provisions for potential contract termination if the contractor is found to be non-compliant with established guidelines. The document is intended to be executed through the Arizona Procurement Portal by the Procurement Officer and the contractor.
