About this file

This document is a Contract Amendment (Amendment No. 3) issued by the Arizona Department of Environmental Quality (ADEQ) for a Statewide Lead Service Line Identification and Inventory Services contract with CDM Smith, Inc. The contract (No. CTR065872) involves adding five new Contractor Key Personnel to the existing contract: Sam Agniel, Becky Dunavant, Michael Higuchi, Rovi Porter, and Kendall Galvez.

The amendment appears to be a unilateral acknowledgement of a contractor's approved change request, specifically expanding the team of key personnel who will support lead service line identification and inventory services. The added team members bring diverse expertise in environmental engineering, water resources, project management, and technical analysis, with backgrounds in areas such as water quality modeling, hydraulic systems, environmental permitting, data analysis, and compliance. The contract amendment maintains all other previously established terms and conditions, focusing solely on the addition of these new key personnel to the existing statewide contract.
