The purpose of this Amendment 001is to:
(1) provide answers to questions;
(2) revise Section I.3.1, FAR Clause 52.217-9; and,
(3) Provide Attachment Number J-2: Past Performance Questionnaire
A. Answers to Questions
Q1. Is this contract subject to the Service Contract Act?
A1. No, this contract is not subject to the Service Contract Act.
Q2. FAR SubPart 52.217-9 is included; however, FAR SubPart 52.217-8 was omitted. Should 52.217-8 -
- Option to Extend Services be included in this RFQ?
A2. FAR 52.21708 is not required for this RFQ. Additionally, Amendment 001 corrects Section I.3.1, FAR 52.217-9 (c) to read 60 months instead of 66 months.
Q3. What are the roles and responsibilities of the required Program Manager listed in the Key Personnel section? Are there any training, certification, education, or skill requirements for this labor category?
A3. As stated in Section 2.2.1, the Program Manager shall serve as the primary point of contact to
CSOSA in the performance of this contract. The Program Manager shall have the authority to fully represent the Contractor in all contractual and technical matters. The Program Manager will be a Key Person. There are no specific training, certification, education, or skill requirements for this labor category.
Q4. The J-2 attachment was not included in the RFQ. Please provide a copy.
A4. A copy of Attachment Number J-2: Past Performance Questionnaire is included as part of
Amendment 001.
Q5. Will the Government provide the internal delivery carts that will be used to deliver the meals to each unit or does the successful offeror need to provide the internal delivery carts for use during the delivery to each unit?
A5. The food transporting carts used to deliver meals to each RSC unit will be the responsibility and property of the contractor.
Q6. Does the Government require a signature of Registered Dietician on the approved menu when submitting for Government approval? If not, what type of proof is required to certify the menu?
A6. The Government requires proof that all menus are approved by a registered dietician who fulfills the requirements of Section 2.2.2 of the Solicitation. The offeror’s proposal should specify how proof of dietician approval will be provided.

Q7. Are there any height/length/ weight/class restrictions for the delivery vehicle to be taken into consideration for the delivery of the meals to the facility?
A7. No, there are no height/length/ weight/class restrictions for the delivery vehicle.
B. Section I.3.1 is revised to read as follows:
“I.3.1 FAR 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 60 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 60 months.”

Attachment Number J‐2: PAST PERFORMANCE QUESTIONNAIRE

1. The Contractor in A below has identified the Questionnaire Respondent as someone for whom the Contractor provided a service similar to the service requirement in paragraph 2.
2. The Contractor is submitting a proposal to the Court Service and Offender Supervision Agency
(CSOSA) to provide the following: The Contractor shall provide all necessary labor, supervision, supplies, materials, equipment, facilities, and transportation required to provide catered meal service to the CSOSA RSC residents in accordance with the requirements specified in the
Statement of Work (SOW), on a firm‐fixed price per meal basis. The catered meal service will include breakfast, lunch, dinner and an evening snack, seven (7) days per week, 52 weeks per year, for up to 102 RSC residents.
3. This Questionnaire should be completed by the individual most knowledgeable of the
Contractor’s day‐to‐day operations and overall condition of the supplies/services provided under the referenced contract. However, that individual is encouraged to supplement their knowledge of the Contractor’s performance with the judgement and observations of others within their organization, as applicable.
4. Handwritten responses are sufficient, however, please ensure they are legible.
5. Please provide explanatory narratives for as many responses as possible. These narratives need not be lengthy, just descriptive. Space for narrative comments is included on the last page of the questionnaire. If more space is required, additional pages may be added to this questionnaire. Please clearly identify which evaluation factor a narrative applies to.
6. As this questionnaire relates to an ongoing source selection for “Re‐entry & Sanctions Center
Foodservice,” we request that all information provided in this questionnaire be safeguarded against unauthorized disclosure.
