CRSU-18-36 - Cheyenne River Health Center (CRHC) – Response to Questions 8/27/18

Via e-mail

1. On page 10 of the RFQ it mentions an orientation. Will the Contractor Personnel (CP) be paid for attending the Orientation?

Response: Yes

2. Is the CP allowed to work weekends only? Or, is the CP required to work weekdays?

Response: Weekdays

3. Is this a new requirement?

Response: No

4. Is there an incumbent? If so can you provide the incumbents name and contract number?

Response: BH Surgical Hospital, Rapid City, SD

5. How does the CP know the availability of housing through IHS?

Response: CRHC does have housing depending on availability. There is a local motel, Cheyenne River Motel, Eagle Butte, SD

6. In reference to the subject solicitation, I would like to know if the Audiologist is required to have a SD license; or if out of state licenses are allowed?

Response: SD or Out of State License are Acceptable.

7. Can the two days per month be consecutive days?

Response: Yes

8. Is there an incumbent?

Response: Yes, see question 4.

9. Is it necessary that the Audiologist have Board certification? Whereas, she/he has a state license and ASHA certification?

Response: The PWS states that Board Certification is favorable not required.

10. On page 3 of the RFQ it states that candidate must include their DOB on the resume. Is this necessary? As this request usually falls into the category of discriminatory questions.

Response: Yes, DOB is used to verify identity.
