Text version
In accordance with Executive Order 13991, Protecting the Federal Workforce and Requiring Mask-Wearing, and guidance issued by the Safer Federal Workforce Task Force and the U.S. Department of Homeland Security Federal Protective Service, HHS OIG is implementing the following COVID-19 vaccination protocols for all contractors working onsite in Federal buildings. Beginning on Friday, September 3, 2021, HHS OIG contractors will be required to attest to their COVID-19 vaccination status using the attached Certification of Vaccination form when accessing HHS OIG facilities.
The Safer Federal Workforce certification of vaccine form must be completed by each contractor and kept on their person while at an HHS OIG site location. The link to this form is below and a copy is attached:

The form provides four potential responses: (1) I am fully vaccinated (as defined in the form); (2) I am not fully vaccinated (as defined in the form); (3) I have not been vaccinated; (4) I decline to respond. If the contractor selects any option on this form other than “I am fully vaccinated,” they must have on their person COVID-19 test results showing a negative test result for COVID-19 within the past 72 hours in order to access an HHS OIG facility. Contractors should carry the applicable document(s) at all times while onsite and be prepared to show them if required by HHS OIG Federal employees or designated building security staff. The government will not retain copies of the documents.
