About this file

This is a Cost Proposal document for IFB Contract 25-0085a, a forklift preventative and corrective maintenance services contract. The contract encompasses a one-year initial term with two optional renewal years (Years Two and Three). The scope of work includes biannual preventive maintenance services consisting of inspection, operational testing, belt and hose changes, belt and timing adjustments, tire inflation, fluid changes, filter cleaning and replacement, component lubrication, operational testing, and documentation of completed work. Additionally, the contract covers corrective maintenance services for operational malfunctions or failures, fluid or fuel leaks, tire replacement, and battery service or replacement. Materials required include replacement parts, lubricants, cleaning supplies, safety equipment, tools, batteries and chargers, and diagnostics equipment. The proposal structure accounts for labor costs, materials costs, and other miscellaneous charges across all three contract years.

The cost proposal is organized into labor, materials, and other service categories with hourly rates for corrective maintenance services, minimum call-out charges per service request, replacement parts and supplies costs, equipment upgrades, and storage and handling equipment expenses. The call-out charge policy specifies that a single call-out fee covers the technician's travel, mobilization, diagnostic assessment, and all repair labor performed during an on-site visit, subject to a minimum labor hour requirement, with no additional transportation charges for work completed during the same visit. If repairs cannot be completed due to timing or parts availability, the vendor will return at no additional call-out or travel cost provided the follow-up visit is scheduled during the initial service call. The document presents blank fields for bidders to enter unit costs and total amounts for materials and labor services across all three contract years.
