About this file

The document is a Cost Response Template for a State and Local Contract Opportunity issued by the Executive Office of Health and Human Services (EOHHS) of Massachusetts. The contract seeks to establish a Master Agreement for at-home and on-site self-administered COVID-19 testing and related services, with the intent to contract with Vault Medical Services, P.A. and Everlywell, Inc. The template requires vendors to provide detailed pricing and service offerings, including specimen collection methods, purchasing and delivery options, return shipping, supervision requirements, minimum and maximum order volumes, all-inclusive costs per test, age restrictions, lead times, and turnaround times. The template also includes example completed rows showcasing different service offerings and pricing.

The related State and Local Contract Opportunity is a Due Diligence Notice, indicating EOHHS' intent to contract with the two vendors mentioned, and providing an opportunity for other interested and qualified vendors to submit for consideration under the Master Agreement. The contract opportunity does not provide specific details on quantities, response dates, award dates, or contract term.
