About this file
This is a Contractor Certification Statements form required for bidders on the NODC Roll-Up Counter Shutter Replacement project in Ohio. The form mandates that bidders claiming eligibility for domestic source end products preference, Ohio preference, or Veteran Friendly Business Enterprise (VBE) status must complete the appropriate certification sections to receive the associated bid preferences. Bidders must certify the origin of products and services being offered, specify whether manufacturing occurs in the United States or meets the 50% domestic component threshold, and declare Ohio manufacturing or significant economic presence within the state. The form also requires VBE certification status as defined in Ohio Administrative Code.
The certification statements establish three distinct preference categories under Ohio law: Domestic Preference under Revised Code 125.11, Ohio Preference under Revised Code 125.09 with requirements for significant economic presence including Ohio tax payment, Secretary of State registration, and employee thresholds in Ohio or border states, and Veterans Preference for certified VBE entities under Revised Code 9.318. Bidders must accurately complete the appropriate certifications or risk disqualification and potential debarment if false or misleading information is submitted. The state reserves the right to clarify certification information during the evaluation process.
