About this file

This is a Contractor Certification Statements form from the State of Ohio that establishes compliance requirements for bidders seeking preference designations on state procurement opportunities. The document does not specify particular products, services, quantities, deadlines, or contract terms, as it functions as a standardized certification template applicable across multiple procurement actions. Instead, it outlines three distinct preference categories that bidders may claim: Domestic Source End Products under the Buy American Act, Ohio preference under Buy Ohio requirements, and Veteran Friendly Business Enterprise status. Bidders must complete the appropriate certification section to qualify for the applicable preference, with false or misleading submissions resulting in immediate disqualification and potential legal consequences including debarment.

The certification requirements address specific sourcing and economic presence criteria depending on the preference claimed. For Domestic preference, bidders must identify where products are mined, raised, grown, produced, or manufactured, with options to claim domestic origin or demonstrate that at least 50 percent of component costs originate within the United States. For Ohio preference, bidders must certify in-state production or significant economic presence demonstrated through payment of Ohio taxes, registration with the Ohio Secretary of State, and employment thresholds of either ten or more employees in Ohio or border states or seventy-five percent or more employees in those regions. Border state bidders from Kentucky, Michigan, New York, Pennsylvania, and Indiana may also qualify under specific provisions. For Veteran preference, bidders must hold certification as a Veteran Friendly Business Enterprise as defined by Ohio Administrative Code. The state reserves the right to clarify any submitted information during evaluation.
