About this file
This document is a Contract Amendment (Amendment #4) for Contract #CTR071427 involving the State of Arizona Department of Child Safety (ADCS), located in Phoenix, Arizona. The amendment was issued by the Office of Procurement and involves modifications to the existing contract's Special Terms and Conditions and Scope of Work.
The amendment addresses two primary changes: first, updating Section 4.8.2 to confirm initial contract prices will be honored for a one-year term, and second, restoring Section 9.14 Adherence that was previously omitted. Key revisions include enhanced background check requirements, drug testing protocols, and personnel screening procedures. Specifically, the amendment mandates that all paid and unpaid adults and subcontractors must complete a Level 1 fingerprint card, obtain Central Registry Clearance, and pass background checks prior to hire. Additionally, employees must complete a Background Check Authorization form, and all new hires must be entered into the Department's electronic database before employment. The effective date of this amendment is upon execution in the Arizona Procurement Portal or August 1, 2025, whichever occurs earlier.
