About this file

This is a Bidder's Questionnaire document issued by the San Francisco Recreation and Parks Department (RPD) for a dredging project at SF Marina (CON27-0001). The questionnaire is a qualification assessment tool that the RPD may request from one or more apparent low bidders within 48 hours following bid opening. All questions regarding the project must be submitted by Thursday, July 23, 2026 at 5:00 p.m., with bids due no later than Thursday, July 30, 2026 at 2:30 p.m. The questionnaire requires bidders to provide comprehensive documentation of their experience, qualifications, and responsibility for the project, including detailed information on past projects, safety metrics, and any disqualifications or terminations within the preceding five years.

The questionnaire is organized into four parts addressing general business information, bidder experience, safety performance, and verification. Bidders must submit their California Contractor's License number and classification, DIR registration number, and detailed project history including the six most recently completed public works projects, three largest projects completed within the last three years, and any similar projects. Safety requirements include providing Experience Modification Rates (EMR), Total Recordable Incident Rates (TRIR), Days Away Restricted or Transferred Rates (DART) for the previous three years, Cal/OSHA Form 300A summaries, and disclosure of any citations or violations. The project carries an estimated cost of $900,000 and requires a Class A Contractor's License. Bidders must certify under penalty of perjury that all submitted information is accurate and complete.
