COMBINED SYNOPSIS/SOLICITATION ‘COMBO’:
CATHOLIC YOUTH MINISTRY COORDINATOR
(i) This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
(ii) Solicitation FA5000-24-Q-2421 is issued as a Request for Quote (RFQ) IAW FAR Part 12.
(iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2024- 05, effective 22 May 2024. The DFARS provisions and clauses are those effective 30 May 2024. The DAFFARS provisions and clauses are those in effect to DAFFARS, effective 12 June 2024.
(iv) This is a small business set-aside requirement under the North America Industrial Classification System (NAICS) code: 813110– Religious Organizations with a small business size standard of $13,000,000 dollars.
(v) The Government intends to award a firm-fixed price (FFP) contract for Catholic youth ministry coordinator. Please see Attachment 1, Price Schedule, and Attachment 2, Performance Work Statement for a full description of the Government’s requirement.
(vi) This requirement is for the procurement of Catholic youth ministry coordinator as required and defined for the 673d Air Base Wing (ABW/HC) in Attachment 2 - Performance Work Statement (PWS). The Catholic Youth Ministry Coordinator will coordinate and provide Catholic ministry for teenagers and young adults, with its primary emphasis being the JBER community, as part of the chapel religious program. Coordinator services include providing a community wide religious youth program and supporting youth ministry needs. Additionally, Coordinator services will include a young adult ministry separate from those provided to teenagers. The programs will be sponsored by the chapel community but will attempt to involve students and young adults throughout the JBER installation. The Coordinator will articulate and develop promotional campaigns and activities and maintain records of supplies, materials and attendance (to include youth and volunteers).
(vii) All items shall be delivery FOB destination and the period of performance will be 30 Aug 2024 to 29 Aug 2028 and will follow the timeline below.
Base Year: 30 Aug 2024 - 29 Aug 2025 Option Year 1: 30 Aug 2025 - 29 Aug 2026 Option Year 2: 30 Aug 2026 – 29 Aug 2027 Option Year 3: 30 Aug 2027 – 29 Aug 2028
(viii) FAR 52.212-1 Instructions to Offerors -- Commercial Products and Commercial Services (SEP 2023) is hereby incorporated by reference, with the same force and effect as if it were given in full text. In addition to the following addendum, all terms, and conditions of FAR 52.212-1 remain in effect. The following have been tailored to this procurement and are hereby added via addendum:
1. To assure timely and equitable evaluation of the proposal, the Offerors must follow the instructions contained herein. The proposal must be complete, self-sufficient, and respond directly to the requirements of this solicitation. If awarded the contract the contractor is required to submit a signed copy of the contract confirming receipt of the contractual document within five (5) business days.
2. Specific Instructions: The response shall consist of the following:
a. Submit one completed copy of price schedule (Attachment 1)
(ix) FAR 52.212-2 Evaluation -- Evaluation-Commercial Products and Commercial Services (NOV 2021) this provision has been tailored to this procurement and is provided in full-text below:
(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

Past performance is slightly more important than price.
The price of a six-month extension authorized by 52.217-8, Option to Extend Services, will be added to the total proposed price.
Option to Extend Services formula calculation is discussed below:
The Total Evaluated Price (TEP) is a two-part computation. First, the offerors total proposed price will be determined by adding the total of the base year and each option year identified in the Pricing Schedule. The sum will be the offerors total proposed price. Second, the proposed price (total amount for CLINs 0001-3001) plus the sum of Option to Extend Services clause 52.217-8 formula (shown below) will constitute the TEP.
The formula for the Option to Extend Services amount is as follows:
CLIN totals for Option Year 3 multiplied by .5 = Option to Extend Services amount Evaluating the Option CLINs does not obligate the Government to exercise the Options.
Next the Government will evaluate the proposal for past performance on a satisfactory or unsatisfactory basis, starting with the lowest priced proposal. The government intends to verify past performance by utilizing information found in SAM.gov as well as other government systems. The government may contact the owning organization of any recent and relevant requirements discovered to collect a verbal performance assessment. The Satisfactory and Unsatisfactory definitions are as follows:
Satisfactory: Owning organization states contractor has been performing within contractual obligations and or SAM.gov or other government systems contain no indicators of unsatisfactory performance.
Unsatisfactory: Owning organization states contractor has not been performing within contractual obligations and shows no intent to remedy the performance issues and or negative performance indicators exist within SAM.gov or other government systems.
Offerors are cautioned to submit sufficient information and, in the format, specified in 52.212-1, Instructions to Offerors – Commercial Items, of this solicitation. Offerors may be asked to clarify certain aspects of their quotes. Exchanges conducted to resolve minor or clerical errors will not constitute discussions and the contracting officer reserves the right to award a contract without the opportunity for quote revision. The Government intends to award a contract without discussions with respective offerors, however, will reserve the right to conduct them if deemed in its best interest.
Award will be made once a price fair and reasonable determination has been made IAW Far Part 13.106-3.

(x) FAR 52.212-3, Offeror Representations and Certifications—Commercial Products and Commercial Services (FEB 2024), Each vendor shall ensure the provision at FAR 52.212-3 including its Alternate I, Offeror Representations and Certifications—Commercial Products and Commercial Services (FEB 2024), are updated at www.sam.gov. Failure to obtain SAM registration will result in submissions not being considered for award.
(xi) The clause at FAR 52.212-4, Contract Terms and Conditions – Commercial Items (NOV 2023), is hereby incorporated by reference, with the same force and effect as if it were given in full text.
(xii) The clause at FAR 52.212-5 (REF for BELOW), Contract Terms and Conditions Required To Implement Statutes Or Executive Orders – Commercial Products and Commercial Services (FEB 2024), is hereby incorporated by reference, with the same force and effect as if it were given in full text. Additionally, the following clauses apply to this acquisition:

52.217-8 -- Option to Extend Services (NOV 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor at least 15 days prior to expiration of the current contract period.

52.217-9 -- Option to Extend the Term of the Contract (MAR 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor up to 30 days prior to current contract period expiration; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 4 years 6 months.

252.219-7012 Competition for Religious-Related Services. (APR 2018)
(a) Definition. As used in this provision—
“Nonprofit organization” means any organization that is—
(1) Described in section 501(c) of the Internal Revenue Code of 1986; and
(2) Exempt from tax under section 501
(a) of that Code.
(b) A nonprofit organization is not precluded from competing for a contract for religious-related services to be performed on a United States military installation notwithstanding that a nonprofit organization is not a small business concern as identified in FAR 19.000(a)(3).
(c) If the apparently successful offeror has not represented in its quotation or offer that it is a small business concern identified in FAR 19.000(a)(3), as appropriate to the solicitation, the Contracting Officer will verify that the offeror is registered in the System for Award Management database as a nonprofit organization.

DAFFARS 5352.201-9101 PACAF Ombudsman (July 2023)
(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern.
(b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).
(c) If resolution cannot be made by the contracting officer, the interested party may contact the ombudsman:

(d) The ombudsman has no authority to render a decision that binds the agency.
(e) Do not contact the ombudsman to request copies of the solicitation, verify offer due date, or clarify technical requirements. Such inquiries shall be directed to the contracting officer.

5352.242-9001 Common Access Cards (CAC) for Contractor Personnel
(a) For installation(s)/location(s) cited in the contract, contractors shall ensure Common Access Cards (CACs) are obtained by all contract or subcontract personnel who meet one or both of the following criteria:
(1) Require logical access to Department of Defense computer networks and systems in either:
(i) the unclassified environment; or
(ii) the classified environment where authorized by governing security directives.
(2) Perform work, which requires the use of a CAC for installation entry control or physical access to facilities and buildings.
(b) Contractors and their personnel shall use the following procedures to obtain CACs:
(1) Contractors shall provide a listing of personnel who require a CAC to the contracting officer. The government will provide the contractor instruction on how to complete the Contractor Verification System (CVS) application and then notify the contractor when approved.
(2) Contractor personnel shall obtain a CAC from the nearest Real Time Automated Personnel Identification Documentation System (RAPIDS) Issuing Facility (typically the local Military Personnel Flight (MPF)).
(c) While visiting or performing work on installation(s)/location(s), contractor personnel shall wear or prominently display the CAC as required by the governing local policy.
(d) During the performance period of the contract, the contractor shall:
(1) Within 7 working days of any changes to the listing of the contract personnel authorized a CAC, provide an updated listing to the contracting officer who will provide the updated listing to the authorizing government official;
(2) Return CACs in accordance with local policy/directives within 7 working days of a change in status for contractor personnel who no longer require logical or physical access;
(3) Return CACs in accordance with local policy/directives within 7 working days following a CACs expiration date; and
(4) Report lost or stolen CACs in accordance with local policy/directives.
(e) Within 7 working days following completion/termination of the contract, the contractor shall return all CACs issued to their personnel to the issuing office or the location specified by local policy/directives.
(f) Failure to comply with these requirements may result in withholding of final payment.
(xiii) No additional contract requirements apply to this acquisition
(xiv) No Defense Priorities and Allocations System (DPAS) assigned rating applies to this acquisition.
(xv) NOTICE TO ALL INTERESTED PARTIES: Please forward all questions to McKenna Hendrix no later than 12:00 p.m. Alaska Daylight Time on 12 August 2024. A site visit will not be conducted for this requirement.
An amendment will be issued answering all questions received, providing the Government’s answers. Quotes are due no later than 4:00p.m. Alaska Daylight Time on 14 August 2024 via email to mckenna.hendrix@us.af.mil.
