QUALITY ASSURANCE SURVEILLANCE PLAN

Equipment Maintenance of Welding/Cutting Machines

1. Vision

Advanced Design and Manufacturing Division (ADM) and top management are committed to meeting applicable requirements and expectations by providing high quality products, services, and systems solutions within budget and schedule. ADM is committed to continuously improve the Quality Management System.

2. Mission

U.S. Army Combat Capabilities Development Command, Chemical Biological Center, ADM located at 8500 Otto Road, Building E4301, Aberdeen Proving Ground, MD, 21010, providing product development and prototyping inclusive of engineering, manufacturing, and systems solutions for the Department of Defense, other government agencies, and industrial customers.

3. Purpose

Yearly maintenance and supply of gasses contract for government owned equipment in ADM located in Building E4301.

3.1. This Quality Assurance Surveillance Plan (QASP) is a government-developed document used to determine if the contractor's performance meets the performance standards contained in the contract. The QASP establishes procedures on how this assessment/inspection process will be conducted. It provides the detailed process for a continuous oversight process:

3.1.1. The Government will use the QASP which provides a systematic method to evaluate the services that the Contractor is required to furnish, but not the details of how the Contractor accomplishes the work.

3.1.2. The QASP has been developed by the requiring activity. It is designed to provide direction to personnel performing contract surveillance activities. Personnel surveying the contract terms and conditions and PWS requirements will periodically review the QASP throughout the life of the contract.

3.2. The contractor is responsible for implementing and delivering performance that meets contract standards using its Quality Control Plan. The QASP provides the structure for the government's surveillance of the contractor's performance to assure that it meets contract standards. It is the government's responsibility to be objective, fair and consistent in evaluating contractor performance.

3.3. The QASP is not part of the contract nor is it intended to duplicate the contractor's quality control plan. This QASP is a living document. Flexibility in the QASP is required to allow for an increase or decrease in the level of surveillance necessary based on contractor performance.

3.4. The government may provide a copy of the QASP to the contractor to facilitate open communication. In addition, the QASP should recognize that unforeseen or uncontrollable circumstances might occur that are outside the control of the contractor.

Bottom line, the QASP should ensure early identification and resolution of performance issues to minimize impact on mission performance.

4. Authority

Authority for issuance of this QASP is provided under Part 46 of the Federal Acquisition Regulation, Inspection of Services clauses, which provide for inspection, acceptance and documentation of the service called for in the contract or order. This acceptance is to be executed by the contracting officer or a duly authorized representative.

5. Roles and Responsibilities

5.1. The Contractor shall provide maintenance and labor for government owned equipment.

5.2. This shall include an annual preventative maintenance on 9 welding machines and manifolds and gas handling equipment, and supply Nitrogen & Oxygen Manifolds, Welding Shielding Gasses and completed in regular scheduled workdays as needed, as mutually agreed upon by the Government and contractor. This service includes all the major maintenance/safety checks that are recommended by the factory, to include:

Annual inspection of all hose, cables and electrical connections, Annual Preventative Maintenance (PM) to be performed factory trained personnel. To include two preventative maintenance inspections per year.

5.3. Contracting Officer Representative. The Contracting Officer Representative (COR) is responsible for quality assurance guidance and to ensure that contract quality requirements, provisions, standards, and thresholds are defined, practical, enforceable, necessary, and verifiable.

5.4. The COR evaluates and documents contractor performance in accordance with the QASP and PWS.

5.5. The COR notifies the Contracting Officer of any significant performance deficiencies using a Contractor Discrepancy Report with supporting documentation.

5.6. The COR maintains surveillance documentation.

5.7. The COR recommends improvements to the QASP and PWS throughout the life of the contract.

5.8. Contracting Officer. The Contracting Officer is responsible for safeguarding the interests of the United States Government in contractual relationships. Only the Contracting Officer is authorized to bind the Government and then, only to the extent of the authority delegated to them through the issuance of a warrant.

5.9. The Contracting Officer delegates authority for inspection and/or acceptance in accordance with the terms of the contract.

5.10. The Contracting Officer informs the contractor of the names, duties, and limitations of authority for all quality assurance personnel assigned to the contract.

6. Performance Requirements and Method of Surveillance This section describes the special requirements for this effort. The following sub-sections provide details of various considerations on this effort.

6.1 Contract Surveillance

The goal of the QASP is to ensure that contractor performance is effectively monitored and documented. The COR's contribution is their professional, non-adversarial relationships with the KO, PM and the contractor, which enables positive, open and timely communications. The foundation of this relationship is built upon objective, fair, and consistent COR evaluations of contractor performance against contract requirements. The COR uses the methods contained in this QASP to ensure the contractor is in compliance with contract requirements. The COR function is responsible for a wide range of surveillance requirements that effectively measure and evaluate the contractor's performance. Additionally, this QASP is based on the premise that the contractor, not the government, is responsible for management and QC/QA actions to successfully meet the terms of the contract.

Surveillance Matrix

6.2.1. When the Contractor arrives, he will be escorted to the area where the equipment is located.

6.2.1.1 After maintenance is completed, a government employee will verify the service report to ensure all maintenance was completed in accordance with the contract. He will then sign-off on the service report and provide to the COR.

6.2.1.2 The COR will maintain a record of services, parts, maintenance, and invoices provided by the

Contractor.

6.2.1.3 When the invoice is submitted in WAWF, the COR will inspect the invoice for accuracy and accept for payment in a timely manner.

6.2.2. The Surveillance Matrix Table 1 is the list of performance objectives and standards that must be performed by the contractor. This matrix details the method of surveillance the COR will use to validate and inspect these performance elements. Inspection of each element will be documented in the COR file.

-Record of services, parts, maintenance and invoices provided to

COR.

- Service reports signed by Contractor and COR.

-Completed copy of the service report provided to the Manufacturing Branch Chief and COR.

Verify accuracy of contractor invoice against PM and service reports before payment is authorized.

6.2.3. Performance objectives define the desired outcomes. Performance Standards define the level of service required under the contract to successfully meet the performance objective. The inspection methodology defines how, when, and what will be assessed in measuring performance. The Government performs surveillance, using this QASP, to determine the quality of the contractor's performance as it relates to the performance element standards.

The matrix should be used to form the foundation of the COR's inspection checklist. In evaluating the quality of contractor’s performance, the following performance ratings may be used.

Table 2. Performance Ratings

Rating

Criteria

Exceptional Performance meets contractual requirements and exceeds many to the Government’s benefit. The element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.

Very Good Performance meets contractual requirements and exceeds some to the Government’s benefit. The element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.

Satisfactory Performance meets contractual requirements. The element being assessed contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.

Marginal Performance does not meet some contractual requirements. The element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions.

Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The element being assessed contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.

The Government intends to utilize a Quality Assurance Surveillance Plan (QASP) to monitor the quality of the Contractor’s performance in relation to performance objectives. The Government will finalize the QASP immediately following award, and the COR will provide a copy of the final QASP to the contractor. The QASP is a living document and may be updated by the Government as necessary.
