Statement of Work
Title: Donor-Derived Cell-Free DNA (dd-cfDNA) Testing Services
1. Introduction
The James J. Peters Bronx VA Medical Center (JJP VAMC) requires comprehensive donor-derived cell-free DNA (dd-cfDNA) testing kits and laboratory services to support the Renal Transplant Program. These services are intended to support post-transplant surveillance for kidney transplant recipients.
2. Background and Objective
Renal transplantation is a critical therapeutic option for patients with end-stage renal disease (ESRD). Ongoing post-transplant monitoring is essential to assess graft health, detect early signs of rejection, and guide timely clinical decision-making. Traditional monitoring methods, such as serum creatinine levels and invasive biopsies, may not detect subclinical graft injury promptly.
Donor-derived cell-free DNA (dd-cfDNA) testing is a non-invasive molecular diagnostic method that quantifies donor DNA fragments in a transplant recipient’s bloodstream.
Elevated levels may indicate allograft injury or immune-mediated rejection, allowing for earlier clinical intervention.
Revision: 03
RFQ: 36C24226Q0287
The objective of this contract is to support the JJP VAMC Renal Transplant surveillance program by providing dd-cfDNA testing services that:
• Deliver reliable and accurate test results within 72 hours of specimen receipt at the testing laboratory.
• Facilitate early detection of allograft injury or immune activation.
• Ensure secure handling, transmission, and confidentiality of patient information.
• Provide training and clinical interpretation support to VA staff upon request.
These services will enable VA clinicians to make timely, evidence-based decisions, reduce reliance on invasive diagnostic procedures, and improve transplant outcomes.

d. Packaging must comply with Department of Transportation (DOT) UN3373 and International Air Transport Association (IATA) regulations for biological materials.
e. Contractor shall maintain chain-of-custody documentation for all shipments.
• The VA Staff will ensure that all donor-derived cell-free DNA (dd-cfDNA) testing kits are secured and, in a temperature-controlled package to ensure integrity of the specimen collection from JJP VAMC to the Contractor’s testing facility.
• Laboratory processing and analysis of specimens must be from a certified laboratory environment.
• Contractor shall secure transmission of test results via fax or a secure encrypted protected email.

Therefore, the Offeror’s initial offer should contain the Offeror’s best terms. However, the Government reserves the right to conduct discussions, if necessary. The Government may reject any or all offers if such action is in the public interest, accept other than the lowest offer, and waive informalities and minor irregularities in offers received.

2. FAR 52.212-2 Evaluation-Commercial Products and Commercial Services: In accordance with RFO FAR 12.201-1 and FAR 12.201-2 when appropriate, the contracting officer may (1) Insert the provision at 52.212-2, Evaluation-Commercial Products and Commercial Services, in solicitations for commercial products or commercial services.
(a) Evaluation factors. The Government will award a contract resulting from this solicitation to the responsible Offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s). Quotations may exceed minimum requirements of the solicitation. The Government reserves the right to select a quotation that provides benefit to the Government that exceeds the minimum requirements of the solicitation but is not required to do so. Each response must meet the minimum requirements of the solicitation. The Government is accepting alternate quotations.
However, the Government may evaluate quotations using the comparative evaluation, where quotations will be compared to one another to determine which provides the best benefit to the Government.
The following factors will be used to evaluate offers:
Factor 1: Technical Capability
The Government will evaluate the extent to which the Offeror demonstrates in their capability statement the ability to successfully perform all requirements of the Statement of Work (SOW). Maximum 10 pages.
mailto:Mariangie.Rivera@va.gov https://www.acquisition.gov/far/part-52#FAR_52_212_2
Revision: 03
RFQ: 36C24226Q0287
The evaluation will assess the Offeror’s:
1. Technical Approach o Understanding of donor-derived cell-free DNA (dd-cfDNA) testing requirements for kidney transplant surveillance.
o Ability to meet required turnaround times (e.g., 72 hours from specimen receipt).
o Procedures for specimen handling, testing, result validation, and reporting.
2. Laboratory Qualifications and Compliance o Evidence of current CLIA certification and CAP accreditation.
o Compliance with applicable federal, VA, and information security requirements (e.g., HIPAA, VA Handbook 6500).
o Participation in External Proficiency Testing (EPT) programs.
Failure to submit evidence of current CLIA certification and CAP accreditation will result in rejection of quote.
3. Quality Control and Risk Management o Adequacy of the Quality Control Plan (QCP) to ensure accuracy, reliability, and continuity of services.
o Processes for addressing testing failures, delays, specimen rejections, or service disruptions.
4. Training and Clinical Support o Ability to provide training and clinical consultation support as required by the SOW.
o Availability and adequacy of educational materials and technical assistance.
Offerors will be evaluated to the degree to which they demonstrate a clear, feasible, and compliant technical solution with minimal risk to the Government.
Factor 2: Past Performance
The Government will evaluate the Offeror’s record of performing relevant and recent contracts for similar services.
Past performance will be evaluated based on:
• Relevance: Similarity in scope, complexity, and regulatory environment.
• Recency: Provide two to three performances within the past three (3) years.
• Quality of Performance: Timeliness, accuracy, customer satisfaction, compliance with contract requirements, and responsiveness to issues.
Sources of information may include, but are not limited to:
• CPARS and other Government databases, Revision: 03
RFQ: 36C24226Q0287
• References provided by the Offeror,
• Information obtained from Federal, state, or commercial customers.
Offerors without a record of relevant past performance will not be evaluated favorably or unfavorably.
Factor 3: Price
Price will be evaluated for reasonableness and completeness, not rated adjectivally.
The Government will assess whether:
• Prices are fair and reasonable
• Prices are consistent with the proposed technical approach.
• All required services are included and clearly priced.
Unbalanced pricing may render a proposal ineligible for award if it poses an unacceptable risk to the Government.
“Evaluations of Options"
For purposes of award of this Contract, the Government intends to evaluate the option to extend services under FAR 52.217-8 as follows: The evaluation will consider the possibility that the option can be exercised at any time and can be exercised in increments of one to six months, but not for more than a total of six months during the life of the contract. The evaluation will assume that the prices for any option exercised under FAR 52.217-8 will be those rates in effect under the contract each time an option is exercised under this clause. The evaluation will therefore assume that the addition of the price or prices of any possible extension or extensions under FAR 52.217-8 to the total price for the basic requirement and the total price for the priced options has the same effect on the total price of all proposals relative to each other, and will not affect the ranking of proposals based on price, unless, after reviewing the proposals, the Government determines that there is a basis for finding otherwise. This evaluation will not obligate the Government to exercise any option under FAR 52.217-8
(c) Notice of award. A written notice of award or acceptance of an offer furnished to the successful Offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party.
