Attachment 1: Combined Synopsis-Solicitation for Commercial Products and Commercial Services RFQ: 36C24124Q0030, Hazardous and Radioactive Waste Services
Description This is a combined synopsis/solicitation for commercial products and commercial services prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, “Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services,” as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued.
This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2024-07.
This solicitation is set-aside for 100% Small Business.
The associated North American Industrial Classification System (NAICS) code for this procurement is 562112, with a small business size standard of $47 Million. The FSC/PSC is F108.
The Network Contracting Officer (NCO) 1 – New England is seeking to purchase Hazardous and Radioactive Waste Services as a Blanket Purchase Agreement (BPA).

(STATEMENT OF WORK)
STATEMENT OF OBJECTIVES
1.0 SCOPE OF REQUIREMENT
The Vendor will come to the West Roxbury (WX) and/or Jamaica Plain (JP) VA Medical Centers to pick up and dispose of radioactive waste in the form of any of the following: Low-Level Radioactive Waste (LLRW) created from medical procedures using, but not limited to, Lu-177 and Y-90 (and associated love-lived contaminants from either one), as well as any other common hospital radionuclides etc., and unused radioactive doses, and Liquid Scintillation Counter (LSC) sample vials with the same radionuclides in Ultima Gold .
The quantity of waste would normally be limited to less than two (2) Plastic 55-gallon drums per pickup, and one (1) plastic pale, but may be more depending on frequency of medical procedures and unusual circumstances that may occur (e.g. spill cleanup). Pickups may be scheduled one or two times per year as and if directed. The general description of waste is as follows:
a. Routine Operating Room materials contaminated during the medical procedures; including but not limited to: injection materials, tubing, gauze, pads, gloves, gowns, decontamination materials, etc.
b. Routine Patient Room materials contaminated during patient inhouse stay; including but not limited to: floor coverings, plastic sheeting and sleeves, gowns, absorbent pads (some pads may contain patient excreta), and disposable food service materials.
c. The waste will be double-bagged and contained in a 30-gallon or 55-gallon plastic drum. The container(s) and bags would be supplied by the vendor, (i.e. exchange filled containers with empty ones).
d. The waste may include unused doses packaged in a 5-gallon plastic pale.

1.2 HOURS OF OPERATIONS
Delivery schedule shall occur from 7:00 a.m. to 3:30 p.m., Monday through Friday, excluding Federal Holidays.

DESCRIPTION OF REQUIREMENTS
1.1 GENERAL REQUIREMENTS
a. Contractor must inform the RSO of any special compact permit(s) which must be acquired by VA prior to the disposal of radioactive sealed sources and other LLRW.
b. Contractor shall coordinate with the RSO to schedule for being on-site to supervise packaging and prepare the LLRW for shipping.
c. Contractor shall provide waste containers and the Department of Transportation (DOT) placards for shipping, labeling, and manifesting of the LLRW and radioactive sealed sources.
d. Contractor must provide a 24-hour emergency contact number as required by DOT.
e. Contractor shall dispose of LLRW and radioactive sealed sources within one (1) year of removal from the VA and provide a certificate of disposal to the RSO indicating the waste location and date of disposal.
1.2 SPECIFIC REQUIREMENTS
a. Contractor shall protect personnel, furnishings, equipment & buildings from damage. Contractor shall remove/replace movable items if necessary and shall replace or report any item damaged due to work performed under this contract.
b. Contractor shall not transfer radioactive sealed sources to another waste broker.
c. Contractor must perform > 50% of the work.
d. Contractor must have a Nuclear Regulatory Commission (NRC) or Agreement State license appropriate for the work described and shall be capable of taking possession of the radioactive sealed sources.
e. Contractor must have a current and appropriate liability insurance.
f. Contractor must have direct ownership or lease of the vehicle that will be used to transport the radioactive material.
g. Contractor shall establish a reciprocity agreement if applicable.
h. All litigation for failure to perform the service IAW applicable laws and regulations shall be borne by the Vendor.
1.3 SECURITY REQUIREMENTS
a. The Contractor performing work on-site shall be escorted by the RSO or personnel designated by the RSO to perform the services herein.
b. The A&A requirements do not apply, therefore a Security Accreditation Package is not required.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records-Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

All quotes shall be sent to the Contracting Officer, Tara Dossiema, tara.dossiema@va.gov Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13. Comparative evaluation is the side-by-side comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government.

2. Technical: The offeror’s quote shall be evaluated to determine if the contractor has the capabilities to provide quality supplies in a timely and efficient manner.
