COMBINED SYNOPSIS/SOLICITATION
Combined Synopsis and Solicitation Notice Information
1. This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.
2. Solicitation CNSHQ17T0055 and is being issued as a Request for Quotes (RFQ).
3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular {2005-94 and 2005-95, effective January 19, 2017}. See http://farsite.hill.af.mil/vffara.htm for this information.
4. The associated NAICS code is 561621 and this procurement:

Location of Work: 1004 G Avenue, Vinton, Iowa 52349 Period of Performance: It is anticipated that the period of performance will begin prior to Sept 30, 2017. This purchase order will be for one-year base year with an option for up to four additional one-year optional years, (for a maximum of five years). Option years will be based on several factors, including but not limited to contractor performance and the Corporation’s need for the completion of these tasks in the option years.
Please submit quotes in writing, on letterhead, as soon as possible. Quotes must be submitted in writing, by email signed by a person authorized to negotiate on behalf of the offeror with the government. Please submit your best offer as the selection will be based solely on the written quotes received.

Statement of Work CCTV Security Camera System AmeriCorps National Civilian Community Corps
Requestor Corporation for National and Community Service AmeriCorps National Civilian Community Corps (AmeriCorps NCCC) North Central Region Campus Requestor Location 1004 G Avenue Vinton, Iowa 52349
Requestor Background AmeriCorps NCCC is a residential, team-based, national service program operated by the Corporation for National and Community Service. The AmeriCorps NCCC mission is to strengthen communities and develop leaders through team-based national and community service. The AmeriCorps NCCC mission is accomplished through the engagement of young people ages 18 to 24 in team-based service on projects in local communities. AmeriCorps NCCC operates throughout the United States with Region Campuses in California, Colorado, Iowa, Maryland, and Mississippi.
Purpose AmeriCorps National Civilian Community Corps is seeking an authorized provider to supply and install a CCTV security camera system at its campus in Vinton, IA.
Period of Performance The provider shall provide all materials and initiate installation of the security camera system within 30 days of award of contract and complete installation within 30 days of the initiation of installation.
Service Requirements
1. Provide and install CCTV security camera system.
2. Must present an installation plan outlining all labor, equipment and resources required for cameras at identified locations.
3. Must provide all cameras, switches, DVRs or other required peripheral devices to cover all annotated points
4. Cameras are to be infrared, between 2 to 4 megapixels, with 1080P video - All cameras installed outdoors must be weatherproof.
5. System to include 34 security cameras installed at designated locations within AmeriCorps NCCC campus facilities consisting of the underground tunnels, Old Main, Rice Hall, and Cottage, as indicated on attached campus floor plans with an “X” ( Appendix A).
6. Capability to monitor all cameras via internet by up to three users simultaneously from multiple locations in real time (minimum 30 frames per second – accessible live and via recording).
7. Include additional option for 17 of the same security cameras installed at designated locations in Palmer Hall (Appendix B)
8. Include additional option for 4 of the same security cameras installed at designated locations in the Recreation building (Appendix C)
9. System must possess access for a minimum of three (3) users for online remote viewing for live video and saved, stored video. Storage may be cloud-based.
10. Include an ongoing maintenance plan with annual options for the base year plus four option years.
11. System must contain storage capacity for video to be saved for a period of no less than 20 days. Storage may be cloud-based or hard-wired. If hard / on-site storage is used demonstrate that it is accessible via internet and the storage capacity required to store a minimum of 20 and maximum of 30 days of footage.
12. System must be expandable to allow for the option of adding 20 additional cameras at a later date. Potential future expansion would be as follows (up to 10 additional in Old Main, 4 additional in the Cottage, and 2 each added in Rice, Palmer and the Rec Building).
13. Include all applicable hardware and software requirements, and materials for installation and operation including, but not limited to cameras, servers, and cabling.
14. Vendor shall provide training following the completion of the system installation.
15. Vendor encouraged to conduct a site-visit in advance. The government will make the facility available for walk-through during normal business hours. Some existing Cat 5E wiring and ports exist that may be usable as part of a system.
Support The vendor shall provide training to designated campus staff and technical support for a period of a minimum of 30 days following the completion of the system installation.
7. The provision at FAR 52.212-1, Instructions to Offerors – Commercial applies to this acquisition and is amended as follows:
52.212-1 (ADDENDUM) ADDITIONAL INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS
a. This procurement is a FAR Part 12 acquisition being conducted under FAR Subpart 13.5 Accordingly, simplified acquisitions procedures apply. After receipt of quotes, the Government may without notice, negotiate with and, if desired, seek a quote revision from as many or as few quoters as it, in its discretion, deems appropriate.
b. The term “offeror” or “offer” as used in FAR 52.212-1 shall be understood to mean “quoter” and “quote,” respectively. Further, the term “award” shall be understood to describe the Government’s issuance of an purchase order.
c. The following paragraphs in FAR 52.212-1 shall not apply to this RFQ: b, c, f (except f(4)), and l.
d. The Government will consider all quotes that are timely received and may consider late quotes. Failure of a quote to address any items required in the submission package may make a quote unacceptable.
e. SUBMISSION PACKAGE INSTRUCTIONS
1. The quote shall contain the following:
Factor 1-Technical Factor Volume 1 Technical Quote must address ALL the following technical evaluation factors:
i. Must present an installation plan outlining all labor, equipment and resources required for cameras at identified location.
ii. Must provide all cameras, switches, DVRs or other required peripheral devices to cover all annotated points
iii. Cameras are to be infrared, between 2 to 4 megapixels, with 1080P video - All cameras installed outdoors must be weatherproof.
iv. Must provide the capability to monitor all cameras via internet by up to three users simultaneously from multiple locations in real time (minimum 30 frames per second – accessible live and via recording). Storage may be cloud-based.
v. Must provide the capability to store all security footage for a minimum of 20 days. Storage may be cloud-based. If hard / on-site storage is used demonstrate that it is accessible via internet and the storage capacity required to store between 20 and 30 days of footage.
vi. System must be expandable to allow for the option of adding 20 additional cameras at a later date. Potential future expansion would be as follows (up to 10 additional in Old Main, 4 additional in the Cottage, and 2 each added in Rice, Palmer and the Rec Building).
vii. Vendor shall provide training following the completion of the system installation.
viii. Include an ongoing maintenance plan with annual options for the base year plus four option years.
2. Volume 2-Pricing:
Volume 2 price quote shall contain the following in the table provided. The following pricing guidelines must be adhered to:
i. Price quote which identifies the requested item(s), unit price, and extended price. Also, quotes shall be broken out by but not limited to; labor hours, labor rate, material, other direct cost, travel, per diem, subcontractors, etc. All quoters shall provide a copy of its published labor rates.
ii. Shipping Price (The Government requests that shipping costs be incorporated into the price of each item rather than a separate line for total shipping costs. The Government will spread the shipping costs across all lines upon award if quoted as a separate line item.)
iii. Total Price
iv. Prompt Payment Terms
v. Remittance address, Tax identification Number, DUNS number and Cage Code
vi. Volume 2 must be submitted in the below table format, supporting sheets may be provided, but the below format is to be provided as the overall price quote.
vii. NO ASSUMPTIONS ARE TO BE INCLUDED IN THE PRICING OR TECHNICAL VOLUMES.
All supporting spreadsheets must be submitted separately in Microsoft Excel separated by worksheets.

All quoters must include a completed copy of FAR 52.212-3, Offeror Representations and Certifications – Commercial Items with its offer.
3. SITE VISITS: Site visits may be scheduled between 9am and 11am, EST, and 1pm and 3pm on Monday, September, 18 or Tuesday, September, June 19, 2017. It is the responsibility of the vendor, if you choose to attend, to schedule the site visit appointment with the point of contact provided below:

It should be noted that NO QUESTIONS WILL BE ANSWERED AT THE TIME OF THE SITE VISIT. All questions are to be submitted via email to Marilyne Brooks at mbrooks@cns.gov no later than Wednesday, September 20, 2017 at 4:00 p.m. Eastern Standard Time (EST). Responses to all questions will be provided via amendment to the solicitation.
4. QUESTION DUE DATE: All questions shall be submitted no later than Wednesday, September 20, 2017 at 4:00 p.m. Eastern Standard Time (EST) via email to mbrooks@cns.gov . Questions submitted after this date and time shall not be answered.
5. QUOTE DUE DATE: All technical and price quotes in response to this solicitation are shall be received no later than Friday, September 22, 2017 at 4:00pm EST . Responses received after this date and time will be considered non-responsive, in accordance with (IAW), Federal Acquisition Regulation (FAR) 52.214-7, Late Submissions, Modifications, and Withdrawals of Bids.

6. Offers/quotes submitted in response to this solicitation shall not contain nor be subject to the offeror's/vendor's standard commercial terms and conditions. Any offer/quote submitted in response to this solicitation which includes the offeror's/vendor's standard commercial terms and conditions may be considered a material defect and may be rejected as being non-responsive to the solicitation.
7. Any data previously submitted in response to another solicitation, or market research will be assumed unavailable to the Contracting Officer, and this data must not be incorporated into the technical offer by reference.
8. PAGE RESTRICTIONS:
The quotes submitted must be legible, singled-spaced, typewritten (on one side only), in a font size not smaller than 10 point pitch with a one-inch margin on all sides, on paper not larger than eight and a half by eleven inches and not exceeding the page limits established in this quote. Pages in excess of the individual limitations shall not be read, and the quote shall be evaluated as if the excess pages did not exist.
9. A quoter’s Technical quote will be evaluated in accordance with the evaluation factors for volume 1. Clarity and completeness of the quote are of the utmost importance. The quote must be written in a practical, clear, and concise manner. Each Volume must be clearly marked.
The following page restrictions by Volume are:
Volume 1 - Technical Quote shall not exceed 10 pages Volume 2 - Price Quote shall not exceed 5 pages, Should be submitted separate from Technical quote.
10. Items not included within the page restriction count are as follows: System for Award Management (SAM), cover pages, cover letters, table of contents, blank section dividers, resumes, charts, graphs, and glossary of terms, exhibits, drawings, sample formats, and items of an illustrative nature. Together, any attached charts, graphs, glossary of terms, exhibits, drawings, sample formats, or other illustrative documents shall not exceed 10 pages.
8. EVALUATION CRITERIA
a. BASIS FOR AWARD :
Award will be made to the responsible, technically acceptable quoter, whose quote, conforming to the Request for Quotation, offers the Lowest Price Technically Acceptable on the following factors:
The Vendor will be selected by Lowest Cost Technically Acceptable.
To be considered Technically Acceptable, the vendor must meet ALL of the below evaluation factors and evaluation will be conducted on the basis of information furnished by the offeror or identified in the offer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer.
Factor 1: Technical Quote (Volume 1)

2. Must provide all cameras, switches, DVRs or other required peripheral devices to cover all annotated points

Factor 2: Past Performance Past Performance will be evaluated, for relevancy and recency in relation to the work requested in the SOW, as part of the consideration for award. The Government evaluation will be based on information obtained from Past Performance Information Retrieval System (PPIRS) at www.ppirs.gov.
a. “Recent” is defined as a contract in-progress or completed within the last three (3) years.
b. “Relevant” is defined as a contract that is of similar scope, magnitude, complexity to the requirements as set forth in this solicitation.
The government will assess the relative risks associated with each offeror. Performance risks are those associated with a quoter's likelihood of success in performing the acquisition requirements as indicated by that offeror's record of past performance.
The assessment of performance risk is not intended to be a product of a mechanical or mathematical analysis of a quoter's performance on a list of contracts but rather the product of subjective judgment by the Government after it considers relevant and recent information.
The lack of a relevant performance record may result in an unknown performance risk assessment, which will result in the Quoter being evaluated neither favorably nor unfavorably for past performance and the quoter will receive a neutral rating.
Factor 3: Price (Volume 2) Award will be made to the Lowest Price Technically Acceptable quote.

9. 52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS--COMMERCIAL ITEMS (Apr 2016) The offeror shall complete only paragraphs (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) Web site accessed through http://www.acquisition.gov . If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (r) of this provision.
(a) Definitions. As used in this provision-- “Economically disadvantaged women-owned small business (EDWOSB) concern” means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States and who are economically disadvantaged in accordance with 13 CFR part 127. It automatically qualifies as a women-owned small business eligible under the WOSB Program.
“Forced or indentured child labor” means all work or service—
(1) Exacted from any person under the age of 18 under the menace of any penalty for its nonperformance and for which the worker does not offer himself voluntarily; or
(2) Performed by any person under the age of 18 pursuant to a contract the enforcement of which can be accomplished by process or penalties.
“Highest-level owner” means the entity that owns or controls an immediate owner of the offeror, or that owns or controls one or more entities that control an immediate owner of the offeror. No entity owns or exercises control of the highest level owner.
“Immediate owner” means an entity, other than the offeror, that has direct control of the offeror. Indicators of control include, but are not limited to, one or more of the following: Ownership or interlocking management, identity of interests among family members, shared facilities and equipment, and the common use of employees.
“Inverted domestic corporation,” means a foreign incorporated entity that meets the definition of an inverted domestic corporation under 6 U.S.C. 395(b), applied in accordance with the rules and definitions of 6 U.S.C. 395(c).
“Manufactured end product” means any end product in product and service codes (PSCs) 1000-9999, except—
(1) PSC 5510, Lumber and Related Basic Wood Materials;
(2) Product or Service Group (PSG) 87, Agricultural Supplies;
(3) PSG 88, Live Animals;
(4) PSG 89, Subsistence;
(5) PSC 9410, Crude Grades of Plant Materials;
(6) PSC 9430, Miscellaneous Crude Animal Products, Inedible;
(7) PSC 9440, Miscellaneous Crude Agricultural and Forestry Products;
(8) PSC 9610, Ores;
(9) PSC 9620, Minerals, Natural and Synthetic; and
(10) PSC 9630, Additive Metal Materials.
“Place of manufacture” means the place where an end product is assembled out of components, or otherwise made or processed from raw materials into the finished product that is to be provided to the Government. If a product is disassembled and reassembled, the place of reassembly is not the place of manufacture.
“Predecessor” means an entity that is replaced by a successor and includes any predecessors of the predecessor.
“Restricted business operations” means business operations in Sudan that include power production activities, mineral extraction activities, oil-related activities, or the production of military equipment, as those terms are defined in the Sudan Accountability and Divestment Act of 2007 (Pub. L. 110-174). Restricted business operations do not include business operations that the person (as that term is defined in Section 2 of the Sudan Accountability and Divestment Act of 2007) conducting the business can demonstrate—
(1) Are conducted under contract directly and exclusively with the regional government of southern Sudan;
(2) Are conducted pursuant to specific authorization from the Office of Foreign Assets Control in the Department of the Treasury, or are expressly exempted under Federal law from the requirement to be conducted under such authorization;
(3) Consist of providing goods or services to marginalized populations of Sudan;
(4) Consist of providing goods or services to an internationally recognized peacekeeping force or humanitarian organization;
(5) Consist of providing goods or services that are used only to promote health or education; or
(6) Have been voluntarily suspended.
Sensitive technology—
(1) Means hardware, software, telecommunications equipment, or any other technology that is to be used specifically—
(i) To restrict the free flow of unbiased information in Iran; or
(ii) To disrupt, monitor, or otherwise restrict speech of the people of Iran; and
(2) Does not include information or informational materials the export of which the President does not have the authority to regulate or prohibit pursuant to section 203(b)(3) of the International Emergency Economic Powers Act (50 U.S.C. 1702(b)(3)).
“Service-disabled veteran-owned small business concern”—
(1) Means a small business concern—
(i) Not less than 51 percent of which is owned by one or more service-disabled veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled veterans; and
(ii) The management and daily business operations of which are controlled by one or more service-disabled veterans or, in the case of a service-disabled veteran with permanent and severe disability, the spouse or permanent caregiver of such veteran.
(2) Service-disabled veteran means a veteran, as defined in 38 U.S.C. 101(2), with a disability that is service-connected, as defined in 38 U.S.C. 101(16).
“Small business concern” means a concern, including its affiliates, that is independently owned and operated, not dominant in the field of operation in which it is bidding on Government contracts, and qualified as a small business under the criteria in 13 CFR Part 121 and size standards in this solicitation.
“Small disadvantaged business concern, consistent with 13 CFR 124.1002,” means a small business concern under the size standard applicable to the acquisition, that--
(1) Is at least 51 percent unconditionally and directly owned (as defined at 13 CFR 124.105) by--
(i) One or more socially disadvantaged (as defined at 13 CFR 124.103) and economically disadvantaged (as defined at 13 CFR 124.104) individuals who are citizens of the United States; and
(ii) Each individual claiming economic disadvantage has a net worth not exceeding $750,000 after taking into account the applicable exclusions set forth at 13 CFR 124.104(c)(2); and
(2) The management and daily business operations of which are controlled (as defined at 13.CFR 124.106) by individuals, who meet the criteria in paragraphs (1)(i) and (ii) of this definition.
“Subsidiary” means an entity in which more than 50 percent of the entity is owned—
(1) Directly by a parent corporation; or
(2) Through another subsidiary of a parent corporation.
“Successor” means an entity that has replaced a predecessor by acquiring the assets and carrying out the affairs of the predecessor under a new name (often through acquisition or merger). The term “successor” does not include new offices/divisions of the same company or a company that only changes its name. The extent of the responsibility of the successor for the liabilities of the predecessor may vary, depending on State law and specific circumstances.
“Veteran-owned small business concern” means a small business concern—
(1) Not less than 51 percent of which is owned by one or more veterans(as defined at 38 U.S.C. 101(2)) or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more veterans; and
(2) The management and daily business operations of which are controlled by one or more veterans.
“Women-owned business concern” means a concern which is at least 51 percent owned by one or more women; or in the case of any publicly owned business, at least 51 percent of the its stock is owned by one or more women; and whose management and daily business operations are controlled by one or more women.
“Women-owned small business concern” means a small business concern --
(1) That is at least 51 percent owned by one or more women or, in the case of any publicly owned business, at least 51 percent of the stock of which is owned by one or more women; and
(2) Whose management and daily business operations are controlled by one or more women.
“Women-owned small business (WOSB) concern eligible under the WOSB Program (in accordance with 13 CFR part 127),” means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States.
(b)
(1) Annual Representations and Certifications. Any changes provided by the offeror in paragraph (b)(2) of this provision do not automatically change the representations and certifications posted on the SAMwebsite.
(2) The offeror has completed the annual representations and certifications electronically via the SAM website accessed through https://www.acquisition.gov. After reviewing the SAM database information, the offeror verifies by submission of this offer that the representation and certifications currently posted electronically at FAR 52.212-3, Offeror Representations and Certifications—Commercial Items, have been entered or updated in the last 12 months, are current, accurate, complete, and applicable to this solicitation (including the business size standard applicable to the NAICS code referenced for this solicitation), as of the date of this offer and are incorporated in this offer by reference (see FAR 4.1201), except for paragraphs ____________. (B) The taxpayer is delinquent in making payment. A taxpayer is delinquent if the taxpayer has failed to pay the tax liability when full payment was due and required. A taxpayer is not delinquent in cases where enforced collection action is precluded.
(ii) Examples.
(A) The taxpayer has received a statutory notice of deficiency, under I.R.C. §6212, which entitles the taxpayer to seek Tax Court review of a proposed tax deficiency. This is not a delinquent tax because it is not a final tax liability. Should the taxpayer seek Tax Court review, this will not be a final tax liability until the taxpayer has exercised all judicial appear rights.
(B) The IRS has filed a notice of Federal tax lien with respect to an assessed tax liability, and the taxpayer has been issued a notice under I.R.C. §6320 entitling the taxpayer to request a hearing with the IRS Office of Appeals Contesting the lien filing, and to further appeal to the Tax Court if the IRS determines to sustain the lien filing. In the course of the hearing, the taxpayer is entitled to contest the underlying tax liability because the taxpayer has had no prior opportunity to contest the liability. This is not a delinquent tax because it is not a final tax liability. Should the taxpayer seek tax court review, this will not be a final tax liability until the taxpayer has exercised all judicial appeal rights.
(C) The taxpayer has entered into an installment agreement pursuant to I.R.C. §6159. The taxpayer is making timely payments and is in full compliance with the agreement terms. The taxpayer is not delinquent because the taxpayer is not currently required to make full payment.
(D) The taxpayer has filed for bankruptcy protection. The taxpayer is not delinquent because enforced collection action is stayed under 11 U.S.C. §362 (the Bankruptcy Code).
