Task Assignment Notification Summary

This document is a Task Assignment Notification under DEP Contract No. SW 230, an Indefinite Delivery Vehicle (IDV) administered by the Florida Department of Environmental Protection (FDEP). The task was assigned to Wheelabrator Ridge Energy Inc., a contractor specializing in waste tire management and processing. The assignment involves the remediation of two illegal waste tire sites located in Central Florida: the Turbeville site at 1306 NE 55th Avenue in Ocala, Marion County, and the Newman site at 2525 Old Hickory Tree Road in St. Cloud, Osceola County. The authorization is based on Section 403.709(3)(c) of the Florida Statutes, which authorizes the FDEP to expend funds from the Solid Waste Management Trust Fund for removal of tires from illegal waste tire sites.

The scope of services includes manual retrieval and transportation of an estimated 1,500 tires from the Turbeville site and approximately 2,000 tires from the Newman site to the contractor's processing facility in Auburndale, Florida. The task assignment amount is $14,600, comprising $6,400 for the Turbeville site (estimated 15 tons) and $8,200 for the Newman site (estimated 24 tons). Pricing is itemized by service: loading and transportation, processing of passenger tires at $85 per ton, processing of tires with rims at $125 per ton, and processing of off-road tires at $250 per ton. The contract specifies a 48-hour removal window for each site and mandates that tires be processed or disposed of within two days of collection to control mosquito generation. All tires must be weighed at certified scales before and after transport, with the lower net weight used for invoicing. The contractor must maintain detailed daily reports, truck and trailer logs, and provide final written documentation with weigh tickets accompanying invoices. The performance period runs from July 27 through August 31, 2016, with a fixed fee arrangement and no additional fixed price amounts specified.

TASK ASSIGNMENT SW 230-6

SCOPE OF SERVICES

TURBEVILLE AND NEWMAN WASTE TIRE SITES

Section 403.709(3)(c), Florida Statutes, authorizes the Department to expend funds from the Solid Waste Management Trust Fund to remove tires from illegal waste tire sites according to priorities established by department rule.

The present Scope of Services is to implement site abatement of the Turbeville and Newman Waste Tire Sites. The objective is to accelerate removal of waste tire accumulations, which can serve as breeding grounds for mosquito species capable of communicating these diseases to humans and animals.

I. ABATEMENT AND SITE WORK

A. Tire Removal

1. Tire retrieval from two separate locations. Tires will be manually loaded from the

Turbeville and Newman Waste Tire Sites, unless otherwise directed by DEP’s Project Manager. The Turbeville Site is located at 1306 NE 55th Avenue, Ocala, Florida 34479 and the Newman Site is located at 2525 Old Hickory Tree Road, St. Cloud, Florida 34772.

2. Remove tires from each Site within a 48-hour period. Contractor will bring any equipment onto the Site to remove vegetation from the tires as necessary.

3. The Contractor will practice safe and efficient methods of loading trailers and lacing tires to maximize hauling efficiency. The Contractor will notify DEP’s Project Manager of any loads less than max capacity. If any stockpiles contain materials other than tires, the Contractor must notify DEP immediately.

4. Haul all passenger and truck tires to Wheelabrator Ridge Energy in Auburndale, Florida unless otherwise agreed by DEP’s Project Manager. Any scrap metal salvaged from removing rims from waste tires that are loaded into the trailer shall become the property of the Contractor.

5. Process and/or dispose of tires within 2 days of trailer removal from the collection sites to control elapsed time for adult mosquito generation.

6. All tires leaving the Turbeville and Newman Sites will be weighed at the nearest certified scale before they are transported to Wheelabrator Ridge Energy in Auburndale, Florida. Tires will be weighed again once they arrive at Wheelabrator and the lower of the two net weights will provide the basis for invoicing. Signed weigh tickets must be attached to the invoice. Invoice will be itemized according to the “Fee Schedule” as shown in Attachment D.

Attachment A, Scope of Services

7. All truck and trailer movements will be logged by time, date, and identification numbers.

A detailed mileage log must be kept to support any mileage-based charges.

8. No security will be provided by DEP at this site.

II. PRODUCTION AND QUALITY CONTROL

A. Production Control

1. The Contractor will assign a Project Manager to oversee the daily operations related to this assignment and to serve as a liaison for the site management Project Manager, if any, and DEP's Project Manager. All matters, correspondence, invoices and reports will be directed to DEP’s Project Manager for disposition. DEP will process the invoice within forty (40) days of receipt from the Contractor. DEP's Project Manager is Michell Mason Smith, (850) 245-8721.

2. The Contractor's Project Manager will keep records of abatement progress, all related truck and trailer movements, hours worked, any problems encountered and any time schedule changes anticipated.

3. Final reports will be made in writing and will specify amounts of materials retrieved, transported and weighed, by tons, on a daily basis, hours worked by Contractor, number of transport trucks and other equipment in operation during the week, problems encountered, and any time or schedule changes anticipated. This report and weigh tickets are to be submitted with Contractor’s invoice.

4. If the amount of tires collected from both sites exceeds the task amount, the Contractor must notify DEP’s Project Manager immediately and cease work until task assignment is amended.

B. Quality Control

1. The Contractor's Project Manager will assure that retrieval, transportation, processing, and all other work to be done under this contract by the Contractor will meet the specifications and standards of DEP Contract #SW230, this Scope of Services, and any supplemental specifications provided by DEP.

2. The Contractor's Project Manager will maintain regular communication with DEP’s

Project Manager to coordinate activities and update records.

3. The Contractor will guarantee that all of the material will be delivered to Wheelabrator Ridge Energy and will be processed according to the Contractor's response in conformance with all applicable laws and regulations. Proof will be required. Any other disposal location must be pre-approved by DEP.

4. The Contractor will obtain prior approval for delivery or disposal of material at any other site and will guarantee that such material will be processed and/or disposed of as represented and in conformance with all applicable laws and regulations.
