This is a Task Assignment Notification for DEP Contract No. SW 230, issued by the Florida Department of Environmental Protection (DEP) to Wheelabrator Ridge Energy Inc. for supplemental waste tire management services in Collier County, Florida. The task assignment was executed on June 21, 2016, with Michell Mason Smith serving as the DEP Contract Project Manager and Christopher Eckert as the Contractor's Project Manager. The assignment implements a state and local initiative to remove waste tires that serve as breeding grounds for mosquitoes carrying Zika virus, West Nile Virus, and Eastern Equine Encephalitis in counties subject to medical alerts.
The contract specifies an amnesty event support program scheduled for June 25, 2016, requiring Wheelabrator Ridge Energy Inc. to haul and transport waste tires from two collection sites—the Immokalee Transfer Station and Collier County Fairgrounds—to the contractor's processing facility in Auburndale, Florida. The scope includes transportation of up to eight trailer loads, processing of tires, and disposal services. The fixed fee schedule amounts to $28,200, comprising $4,800 for transportation (eight trailers at $600 per trailer), $8,400 for processing standard tires (120 tons at $70 per ton), and $15,000 for processing tires with rims (120 tons at $125 per ton), based on an estimated 240 total tons. The period of performance runs from June 23 through June 30, 2016. The contractor is required to obtain prior approval for any alternative disposal locations, maintain detailed documentation and weigh tickets, and provide a final report with the invoice. The contractor must also maintain liability insurance of $1,000,000 per occurrence and $2,000,000 annual aggregate, along with workers' compensation coverage.

SCOPE OF SERVICES
SUPPLEMENTAL DEP WASTE TIRE MANAGEMENT PROGRAM – COLLIER COUNTY
Section 403.709(3)(c), Florida Statutes, authorizes the Department to expend funds from the Solid Waste Management Trust Fund to remove tires from illegal waste tire sites according to priorities established by department rule.
The present Scope of Services is to implement a supplemental DEP waste tire management program described in Attachment B, specifically Amnesty Event Support for Collier County. This program is intended to reduce public health risks within Florida counties subject to a medical alert or advisory for Zika, West Nile Virus and/or Eastern Equine Encephalitis. Collier County is under such alert. The objective is to accelerate removal of waste tire accumulations, which can serve as breeding grounds for mosquito species capable of communicating these diseases to humans and animals. The following outline of services is necessarily broad in order to include the range of services that may be required, and it has been structured to parallel the program components.
I. ABATEMENT AND SITE WORK
A. Amnesty Day Support
1. The proposed amnesty collection is scheduled for June 25, 2016, from 8:00 am to 2:00 pm. Contractor will haul trailers to and from the designated locations when requested.
2. Tire retrieval from two separate locations. Tires will be loaded from the Immokalee Transfer Station and the Collier County Fairgrounds on June 25th, 2016, unless otherwise directed by DEP’s Project Manager. The Immokalee Transfer Station is located at 700 Stockade Road, Immokalee, Florida 34142 and the Collier County Fairgrounds is located at 751 39th Avenue, Naples, FL 34120. The contact person in Collier County to coordinate pick up dates is Dilia Camacho, telephone number (239) 252-4281 office and (239) 285-3267 cell phone.
3. Collier County will load the tires into the trailers at both locations, up to a maximum of eight trailer loads. Collier County has been advised that the tires cannot exceed 54” in height and 18” in width; the tires must be free of dirt, grease and hazardous waste;
and no off road tires, foam filled, solid rubber or cut, sectioned or baled tires can be loaded. Tires on rims will be separated.
4. The Contractor will practice safe and efficient methods of loading trailers and lacing tires to maximize hauling efficiency. The contractor notifies DEP’s Project Manager immediately if any stockpiles contain materials other than tires.
5. Haul all passenger and truck tires to Wheelabrator Ridge Energy in Auburndale, Florida unless otherwise agreed by DEP’s Project Manager. Any scrap metal salvaged from removing rims from waste tires that are loaded into the trailer shall become the property of the Contractor.
6. Process and/or dispose of tires within 2 days of trailer removal from the collection site to control elapsed time for adult mosquito generation.
7. There is a certified scale onsite at the Immokalee Transfer Station to be utilized for weighing all trailers loaded at the Immokalee Transfer Station. There will be no scale located at the Collier County Fairgrounds. All trailers leaving the Collier County Fairgrounds will be weighed immediately after leaving the site on the certified scale located at the Immokalee Transfer Station also. Arrangements will be made through Dilia Camacho with Collier County for Immokalee Transfer Station notification of the use of their scales. All trailers will be weighed again on certified scales when they arrive at Wheelabrator Ridge Energy in Auburndale, Florida. The lower of the two net weights will provide the basis for invoicing. Signed weigh tickets must be attached to the invoice. Invoice will be itemized according to the “Fee Schedule” as shown in Attachment D.
8. All truck and trailer movements will be logged by time, date, and identification numbers.
A detailed mileage log must be kept to support any mileage-based charges.
9. No security will be provided by DEP at this site.
II. PRODUCTION AND QUALITY CONTROL
A. Production Control
1. The Contractor will assign a Project Manager to oversee the daily operations related to this assignment and to serve as a liaison for the site management Project Manager, if any, and DEP's Project Manager. All matters, correspondence, invoices and reports will be directed to DEP’s Project Manager for disposition. DEP will process the invoice within forty (40) days of receipt from the Contractor. DEP's Project Manager is Michell Mason Smith, (850) 245-8721.
2. The Contractor's Project Manager will keep records of abatement progress, all related truck and trailer movements, hours worked, any problems encountered and any time schedule changes anticipated.
3. Final reports will be made in writing and will specify amounts of materials retrieved, transported and weighed, by tons, on a daily basis, hours worked by Contractor, number of transport trucks and other equipment in operation during the week, problems encountered, and any time or schedule changes anticipated. This report and weigh tickets are to be submitted with Contractor’s invoice.
B. Quality Control
1. The Contractor's Project Manager will assure that retrieval, transportation, processing, and all other work to be done under this contract by the Contractor will meet the specifications and standards of DEP Contract #SW230, this Scope of Services, and any supplemental specifications provided by DEP.
2. The Contractor's Project Manager will maintain regular communication with DEP’s Project Manager to coordinate activities and update records.
3. The Contractor will guarantee that all of the material will be delivered to Wheelabrator Ridge Energy and will be processed according to the Contractor's response in conformance with all applicable laws and regulations. Proof will be required. Any other disposal location must be pre-approved by DEP.
4. The Contractor will obtain prior approval for delivery or disposal of material at any other site and will guarantee that such material will be processed and/or disposed of as represented and in conformance with all applicable laws and regulations.

TASK ASSIGNMENT SW 230-5
GENERAL DESCRIPTION OF PROGRAM OBJECTIVES
SUPPLEMENTAL DEP WASTE TIRE MANAGEMENT PROGRAM – COLLIER COUNTY
Many Florida counties are currently under medical alert or advisory for potentially serious diseases, namely Zika, West Nile Virus (WNV) and Eastern Equine Encephalitis (EEE).
Both diseases can be communicated to humans by mosquito species known to breed in stagnant water in outdoor containers, such as scrap tires. The Florida Department of Environmental Protection (DEP) and many counties have had effective programs to control and remove waste tire piles, funded by the $1.00/tire fee paid by consumers when purchasing new tires. Large and small stockpiles containing over 14 million waste tires have been removed, processed and constructively utilized since the program was initiated in 1989. Although few major stockpiles remain in the state, DEP is initiating several supplemental programs intended to enhance cooperative efforts by state, county and local governments to remove remaining small waste tire accumulations in counties affected by the emergency order or medical alert.
The proposed program is intended to utilize the strengths of each level of government to accelerate collection, transport and processing of waste tires. DEP has existing contracts with processors that are capable of providing transport trailers, hauling and processing waste tires for constructive applications. Counties and local governments have communication capabilities and sites that can be used as secure collection and loading locations. Merging these capabilities should accelerate removal of waste tires from small accumulations and reduce this breeding ground for dangerous mosquitoes. As a result, the following supplemental program components have been initiated:
(1) Amnesty Day Support – Counties may request support for amnesty days to allow local residents to remove small quantities of tires from local properties. The county will be responsible for: (a) requesting assistance from DEP for a specific time interval and estimated quantity of tires so that DEP can make necessary contractor arrangements;
(b) designating a secure and monitored collection site for receipt of quantities of up to 25 tires per delivery by local residents; and (c) seeing that these tires are efficiently loaded into trailers at these sites. DEP would utilize, and pay for, its contractors to transport, process and properly use or dispose of the tires.
(2) Small Site Assistance – Counties may request assistance in abatement of specifically identified sites containing less than 5,000 tires, subject to prioritization and approval by DEP. DEP will require each site to be examined, estimated and documented by a designated representative prior to approval. Tires from smaller pre-approved sites (less than 1,000 tires) will be transported to a designated collection location and loaded onto transport trailers by the landowner, responsible party or county unless otherwise agreed by DEP. The landowner and/or responsible party must sign an agreement not to accumulate additional tires in exchange for the state waiving cost recovery for transport and disposal costs for as long as the agreement is honored. Larger sites up to 5,000 tires may be considered by DEP for similar treatment under appropriate conditions, and
Attachment B, Program Objectives trailers may be spotted at the site for loading by the responsible party or the county.
DEP will bear the cost of transport and disposal for pre-approved quantities of tires.
(3) Larger Site Access *– State, county and local enforcement groups will coordinate efforts to identify larger sites (over 5,000 tires) and encourage abatement by the responsible parties. If the responsible party is unable or unwilling to abate the site promptly, site access should be requested in writing so that DEP or the county can abate the site.
DEP will use its contractors, or alternative cooperative efforts with the county, and will follow our normal procedures to clean up these sites.
These supplemental initiatives are intended to assist counties and local governments affected by the current medical alert in removal of tires representing a mosquito-breeding hazard to its residents. Requests, questions, and suggestions should be directed to Michell Mason Smith, at 2600 Blair Stone Road, MS 4550, Tallahassee, FL 32399, telephone number (850) 245-8721.
*Larger sites will be handled as they always have been with a separate Task Assignment and Scope of Services for each Large State Waste Tire Site Abatement.
