HW561 DC023L

This Task Assignment authorizes Golder to conduct groundwater monitoring activities in accordance with the December 18, 2020, Scope of Work and Cost Estimate.

Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.

Subtasks 2 – 5: Quarterly Groundwater Monitoring Golder will conduct quarterly groundwater monitoring activities in December 2020, March, 2021, June 2021, and September 2021. Golder will sample the wells and analyze the samples for chlorinated volatile organic compounds (VOCs), dissolved gases, and total organic carbon as outlined in the DEP-approved monitoring plan found in the attached Table 8.

Prior to each sampling event, depth-to-water will be measured. The wells will then be purged, and field parameters will be recorded. The groundwater samples will be collected using low flow sampling methods. Investigation-derived waste (IDW) will be temporarily stored in a 55-gallon drum before being treated with Golder’s aqueous IDW treatment system and disposed of on site.

During the December 2020 groundwater monitoring event, Golder will also conduct soil gas sampling at soil vapor extraction (SVE) wells SVE-1 – SVE-11 and VEW-12 – VEW-14. The samples will be analyzed for carbon monoxide, methane, hydrogen sulfide, and oxygen.

Subtask 6: COVID-19 Related Costs Costs related to the use of extra vehicles so that Golder can maintain social distancing during the COVID-19 pandemic are covered under this subtask.

Subtask 7: Reporting Golder will submit the Annual Groundwater Monitoring Report and ADaPT and GIS deliverables to FDEP.

RE: COST PROPOSAL FOR GROUNDWATER MONITORING

FORMER BEACHES LAUNDRY AND CLEANERS

106 NORTH 6TH STREET, JACKSONVILLE BEACH, FLORIDA

DEP FACILITY ID NUMBER: ERIC_4606

PROPOSED TASK ASSIGNMENT: DC023L

Dear Mr. Grubbs:

Golder Associates Inc. (Golder) is pleased to provide the Florida Department of Environmental Protection (FDEP) with this cost proposal for groundwater monitoring at the above-referenced site. This proposal includes a description of the proposed scope of services, level of effort sheets for each subtask, and a cost estimate form that summarizes the costs of the proposed services. The proposed scope of services is described below.

SUBTASK 1 - PROJECT MANAGEMENT

To facilitate project quality and satisfactory completion of the proposed scope of work, all aspects of this project will be overseen by an experienced project manager. The project manager will be responsible for the schedule, budget, invoicing, project activities, and communications with FDEP and the field team. Project management will include coordination of field activities; resource allocation to confirm the work is conducted by qualified professionals in accordance with the project schedule; cost control and tracking of the budget status; and submittal of project invoices in accordance with FDEP Drycleaning Solvent Cleanup Program requirements. The project manager will oversee the work so that it is conducted in a manner that is consistent with the requirements of the FDEP-approved task assignment.

SUBTASKS 2 THROUGH 5 – QUARTERLY GROUNDWATER MONITORING

Golder completed groundwater injection activities at the site in June 2019. Golder then completed five post-injection, groundwater performance monitoring events in 2019-2020, with the most recent event taking place in

June 2020. In its Annual Injection and Performance Monitoring Report, dated August 2020, Golder recommended continued monitoring of the shallow, intermediate, and deep zones of the surficial aquifer, focusing on areas with high concentration volatile organic compounds (VOCs) exceeding Natural Attenuation Default Concentrations

(NADCs) or in areas where a significant concentration of VOCs were observed. The FDEP approved the proposed monitoring plan via e-mail on November 24, 2020. A copy of the plan is included in the attached Table 8. The attached Figure 5 through Figure 10 from the August 2020 report show the well locations and VOC results for the shallow, intermediate, and deep zones of the surficial aquifer.

Mr. Ben Grubbs FDEPDC023 P13-82530

Florida Department of Environmental Protection December 18, 2020

Golder will mobilize to the site on a quarterly basis (December 2020, March 2021, June 2021, and September 2021) and collect groundwater samples in accordance with the FDEP-approved performance monitoring plan (Table 8).

Prior to groundwater sampling during each event, the water level depth will be measured at each well to be sampled to determine groundwater elevations. Groundwater samples will be collected from the wells in accordance with

FDEP Standard Operating Procedures (SOPs) for Field Activities (DEP SOP-001/01), dated January 2017 (effective

April 16, 2018). Prior to collecting the groundwater sample, each well will be purged and field parameters including temperature, pH, specific conductance, dissolved oxygen, turbidity, and oxidation-reduction potential will be monitored. Purge volumes and field parameters will be recorded on field data sheets, and the field monitoring equipment will be calibrated in accordance with the manufacturer’s recommended procedures. During the sampling event, Golder will inspect the locks and well caps for damage or degradation and replaced, as necessary.

Groundwater samples will be collected using low flow sampling methods. The groundwater samples will be placed in laboratory-supplied containers, labeled with a unique identification number attached to the outside of the sample container, preserved on ice in a laboratory-provided cooler at a maximum temperature of four degrees Celsius, and delivered to a Florida Department of Health and National Environmental Laboratory Accreditation Program-certified fixed-base laboratory along with the completed chain-of-custody documentation to be analyzed in accordance with the attached Table 8.

Golder will temporarily store purge water generated from the groundwater sampling in a 55-gallon steel drum and treat the investigation derived waste (IDW) on site for one hour using Golder’s aqueous IDW treatment system.

Based on the historical IDW characterization data, the treated purge water will be below the regulatory standards.

Golder will spread the treated aqueous IDW on an impervious surface in locations sufficiently distant from any storm water drains to prevent runoff into the drains. The empty 55-gallon drum will be removed from the site.

In addition, during the December 2020 groundwater sampling event, Golder will collect soil gas samples from each of the 14 soil vapor extraction wells that are currently used for passive venting: SVE-1, SVE-2, SVE-3, SVE-4, SVE-

5, SVE-6, SVE-7, SVE-8, SVE-9, SVE-10, SVE-11, VEW-12, VEW-13, and VEW-14. The purpose of the sampling event will be to evaluate the effect of the enhanced bioremediation program on methane generation in soil gas underneath the building. Samples will be collected using an soil vapor extraction sampling box to fill a 1 liter tedlar bag. Carbon monoxide, methane, hydrogen sulfide, and oxygen will then be measured using a RAE Systems

QRAE 3 quad-gas meter. Measurements and calibration records will be recorded in a field notebook. The locations of the soil vapor extraction wells are shown on the figure included in Attachment A.

Interim e-mail deliverables (field documentation, lab reports, and brief data evaluation) will be submitted following the completion of Subtasks 2 through 5.

SUBTASK 6 – COVID-19 RELATED COSTS

This subtask will include any project related costs directly associated with COVID-19 safety precautions.

SUBTASK 7 – REPORTING

The Annual Groundwater Monitoring Report will include a description of the sampling procedures, evaluation of the analytical results, comparison of current to historical data, tabulated summary of the current and historical analytical data, a site map showing the groundwater analytical results, water level elevation information in tables and figures, laboratory analytical data, conclusions, and recommendations. Golder will update the FDEP GIS tables with the new water level data, field parameter measurements, and VOC data; run the FDEP data checker on the completed tables; and submit the updated tables and error reports under this subtask. Additionally, Golder will forward

Automated Data Processing Tool (ADaPT) data deliverables provided by the laboratory for the groundwater sampling event.

HEALTH AND SAFETY

Golder will update the existing site-specific Health and Safety Plan for the field work. The work will be completed in accordance with Golder’s safety policies and procedures. At a minimum, Golder will wear safety glasses, high visibility safety vests, and steel-toed boots. A safety job briefing will be conducted prior to the start of work activities, and anytime the scope of work or site conditions change.

COST AND SCHEDULE

The estimated cost to complete the monitoring is $38,854.27. The LOE/CEF cost sheets are included in Attachment

B. The laboratory subcontractor quote is included in Attachment C. The following table summarizes the proposed costs and deliverable schedule.
