From: Ben Grubbs, Project Manager Date: May 26, 2021 Subject: Negotiation Summary HW561 DC025i

This Task Assignment authorizes Golder to conduct groundwater monitoring activities in accordance with the May 17, 2021, Scope of Work, Cost Estimate, and Bid Documentation.

Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.

Subtask 2: Annual Groundwater Monitoring Golder will collect groundwater samples from monitoring wells SW-4, SW-10, SW-13, SW-15, SW-17, IW-1, IW-4, IW-9, IW-10, IW-13, IW-15, and IW-17. Prior to sampling, depth-to-water will be measured. The wells will then be purged, and field parameters will be recorded. The groundwater samples will be collected using low flow sampling methods and analyzed for chlorinated volatile organic compounds (VOCs). The samples from SW-10, SW-15, IW-1, and IW-15 will also be analyzed for perfluoroalkyl and polyfluoroalkyl substances (PFAS). Costs related to PFAS sampling are covered under Subtask 4 for invoicing purposes. Investigation-derived waste (IDW) will be temporarily stored in a 55-gallon drum. Golder requested bids for IDW transport and disposal from Erwin Remediation, Capitol Environmental, and Clark Environmental. Erwin Remediation was selected as the lowest responsive and responsible bidder.

Subtask 3: COVID-19 Costs Costs related to the use of an extra vehicle so that Golder can maintain social distancing during the COVID-19 pandemic are covered under this subtask for invoicing purposes.

Subtask 4: PFAS Sampling Costs Costs related to sampling for PFAS are covered under this subtask for invoicing purposes.

Subtask 5: Reporting Golder will submit the Annual Groundwater Monitoring Report and ADaPT and GIS deliverables to FDEP.

RE: COST PROPOSAL FOR THE MUSIC MAN (GRACE & COMPANY, INC.)

ANNUAL GROUNDWATER MONITORING

799 LANE AVENUE SOUTH, JACKSONVILLE, FLORIDA

DEP FACILITY ID NUMBER: ERIC_4619

PROPOSED TASK ASSIGNMENT: DC025I

Dear Mr. Grubbs:

Golder Associates Inc. (Golder) is pleased to provide the Florida Department of Environmental Protection (DEP) with this cost proposal for annual groundwater monitoring at the above-referenced site. This proposal includes a description of the proposed scope of services, level of effort sheets for each subtask, and a cost estimate form that summarizes the costs of the proposed services. The proposed scope of services is described below.

SUBTASK 1 – PROJECT MANAGEMENT

To facilitate project quality and satisfactory completion of the proposed scope of work, all aspects of this project will be overseen by an experienced project manager. The project manager will be responsible for the schedule, budget, invoicing, project activities, and communications with DEP and the field team. Project management will include coordination of field activities; resource allocation to confirm the work is conducted by qualified professionals in accordance with the project schedule; cost control and tracking of the budget status; and submittal of project invoices in accordance with DEP Drycleaning Solvent Cleanup Program requirements. The project manager will oversee the work so that it is conducted in a manner that is consistent with the requirements of the DEP-approved task assignment.

SUBTASK 2 – ANNUAL GROUNDWATER MONITORING

Golder will mobilize to the site and collect groundwater samples from 12 monitoring wells. Groundwater samples will be collected from five shallow zone monitoring wells (SW-4, SW-10, SW-13, SW-15, and SW-17), and seven intermediate zone monitoring wells (IW-1, IW-4, IW-9, IW-10, IW 13, IW-15, and IW-17). Figures 5 and 6 from the previous annual monitoring report, which show chlorinated volatile organic compound (VOC) results for the surficial aquifers, are attached. In addition, Golder has selected four of the above-referenced monitoring wells to collect groundwater samples for analysis of perfluoroalkyl and polyfluoroalkyl substances (PFAS). Golder recommends collecting groundwater samples shallow zone wells SW-10 (source well) and SW-15 (downgradient well) and intermediate zone wells IW-1 (source well) and IW-15 (downgradient well). The PFAS groundwater results, from the four monitoring wells, will be used to characterization of the IDW drum for disposal purposes.

Mr. Benjamin Grubbs CX21-465586

Florida Department of Environmental Protection May 17, 2021

Golder will collect the following QA/QC samples during the groundwater sampling event:

Equipment Blank: Golder will collect one equipment blank by pumping (peristaltic pump) PFAS-free water through silicon and/or HDPE tubing and into laboratory-supplied containers.

Duplicate Sample: Golder will collect one field duplicate during the sampling event. The duplicate sample will be collected immediately after collection of the primary field sample. The label for the duplicate sample will be consistent with the primary sample and include “Duplicate” or “Dup” in the sample ID to designate it as a duplicate sample.

Matrix Spike and Matrix Spike Duplicate (MS/MSD) Sample: If provided by the FDEP laboratory, Golder will collect an MS/MSD sample in the same manner and at the same time and location as a primary sample (i.e., additional sample volume). MS/MSD samples will be labeled with the same sample name and time as the primary sample and denoted as MS/MSD samples on the chain-of-custody and sample label.

Prior to groundwater sampling, depth-to-water measurements using an electronic water level indicator to determine groundwater depth. During the sampling event, the locks and well caps will be inspected for damage or degradation and repaired or replaced, as necessary.

Groundwater samples will be collected from the wells in accordance with FDEP Standard Operating Procedures (SOPs) for Field Activities (DEP SOP-001/01), dated January 2017 (effective April 16, 2018) and FDEP SOPs for Sampling with and Analysis of PFAS, dated August 2020. Prior to collecting the groundwater sample, each well will be purged and field parameters including temperature, pH, specific conductance, dissolved oxygen, turbidity, and oxidation reduction potential will be monitored. Purge volumes and field parameters will be recorded on field data sheets, and the field monitoring equipment will be calibrated in accordance with the manufacturer’s recommended procedures.

Groundwater samples will be collected using low-flow sampling methods. The groundwater samples will be placed in laboratory-supplied containers, labeled with a unique identification number attached to the outside of the sample container, preserved on ice in a laboratory-provided cooler at a maximum temperature of four degrees Celsius. The groundwater samples to be analyzed for VOCs will be delivered to a Florida Department of Health and National Environmental Laboratory Accreditation Program certified, fixed-base laboratory, along with the completed chain-of-custody documentation to be analyzed for chlorinated VOCs using EPA Method 8260B. For quality assurance purposes, one trip blank will accompany each sample cooler for laboratory analysis of chlorinated VOCs using EPA Method 8260B. The groundwater samples to be analyzed for PFAS samples will be shipped to the FDEP Central Laboratory along with the completed chain-of-custody documentation. The samples will be analyzed for PFAS by Environmental Protection Agency (EPA) Method 8321B. Upon receipt of the laboratory analytical results, Golder will evaluate the constituent concentrations to determine if off-site notification is required.

Golder will temporarily store the investigation derived waste (IDW) purge water generated from the groundwater sampling event in a 55-gallon steel drum(s). Golder will collect one IDW sample from the drum(s) for analysis of chlorinated VOCs using EPA Method 8260 for waste characterization. For costing purposes, Golder has also assumed that the IDW will be characterized as non-hazardous waste. Following characterization as non-hazardous waste the IDW drum(s) will be picked up and transported to a facility for proper disposal. A non-hazardous waste manifest will be provided by the disposal subcontractor upon delivery to the disposal facility.

Upon completion of Subtask 2, an e-mail deliverable will be submitted and include copies of the field data sheets, equipment calibration forms, field notes, laboratory analytical report, laboratory chain-of-custody documentation, and non-hazardous waste manifest.

SUBTASK 3 – COVID-19 COSTS

This subtask will include any project related costs directly related to the COVID-19 pandemic.

SUBTASK 4 – PFAS SAMPLING COSTS

This subtask will include any project related costs directly related to PFAS sampling.

SUBTASK 5 – REPORTING

The annual groundwater monitoring report will include a description of the sampling procedures, evaluation of the analytical results, comparison of current to historical data, tabulated summary of the current and historical analytical data, a site map showing the groundwater analytical results, water level elevation information in tables and figures, laboratory analytical data, conclusions, and recommendations. Golder will also update the DEP GIS tables with the new water level data, field parameter measurements, and VOC data; run the DEP data checker on the completed tables; and submit the updated tables and error reports under this subtask. Additionally, Golder will forward the Automated Data Processing Tool (ADaPT) data deliverables for the groundwater sampling event provided by the laboratory.

HEALTH AND SAFETY

Golder will update the existing site-specific Health and Safety Plan for the field work. The work will be completed in accordance with Golder’s safety policies and procedures. At a minimum, Golder will wear safety glasses, high visibility safety vests, and steel-toed boots. A safety job briefing will be conducted prior to the start of work activities, and anytime the scope of work or site conditions change.
