HW561 DC002D

This Task Assignment authorizes Golder Associates to conduct annual groundwater monitoring at site.

Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.

Subtask 2: Groundwater Monitoring & IDW Treatment Subtask includes costs and labor related to collection of groundwater samples from site and treatment of investigative derived waste (IDW). The field event is scheduled to take one day comprised of the following activities; collection of depth to water will be measured from site monitoring wells, field parameters for geochemical data, one (1) groundwater samples for analysis of volatile organic compounds via EPA Method 8260B, and purge water treatment.

Subtask 3: Reporting This subtask includes labor and related costs associated with the preparation and submittal of the groundwater monitoring report. Golder will provide conclusions and recommendations; update the Department GIS tables; and submit the ADaPT data deliverables. After negotiations with contractor hours related to report generation have been reduced by six (6) hours.

Original Task Assignment totaled $4,143.34. with 34 labor hours but after negotiations and corrections with contractor costs are reflected as below:

The FDEP project manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $3,520.75 with 27 labor hours.

Golder Associates may invoice following the schedule established in the Deliverables and Invoice Schedule Attachment, which accompanies the Task Assignment Change Order Notification Form.

RE: COST PROPOSAL FOR PRESTIGE CLEANERS

2019 GROUNDWATER MONITORING

2210 NORTH MONROE STREET

TALLAHASSEE, FLORIDA

FDEP FACILITY ID: ERIC_4857

PROPOSED TASK ASSIGNMENT: DC002D

Dear Ms. McClure:

Golder Associates Inc. (Golder) is pleased to provide the Florida Department of Environmental Protection (FDEP) with this cost proposal for groundwater monitoring and reporting at the Prestige Cleaners site located at

2210 North Monroe Street in Tallahassee, Leon County, Florida. This proposal includes a description of the proposed scope of services, level of effort (LOE) sheets for each subtask, and a Cost Estimate form (CEF) that summarizes the costs of the proposed services. The proposed scope of services is described below.

SUBTASK 1 – PROJECT MANAGEMENT

To facilitate project quality and satisfactory completion of the proposed scope of work, all aspects of this project will be overseen by an experienced project manager. The project manager will be responsible for the schedule, budget, invoicing, project activities, and communications with FDEP. Project management will include coordination of field activities; resource allocation to confirm the work is conducted by qualified professionals in accordance with the project schedule; cost control and tracking of the budget status; and submittal of project invoices in accordance with

FDEP Drycleaning Solvent Cleanup Program requirements. The project manager will oversee the work so that it is conducted in a manner that is consistent with the requirements of the FDEP-approved task assignment.

SUBTASK 2 – GROUNDWATER MONITORING AND IDW TREATMENT

As discussed in our November 2018 site summary meeting in Tallahassee, Golder will collect a groundwater sample from monitoring well MW003 and collect depth-to-water measurements from monitoring wells MW001 through

MW005. Figures 3 and 4 from the previous monitoring report in 2015 are attached, which show the potentiometric map and historical volatile organic compound (VOC) results for the surficial aquifer

Prior to groundwater sampling, the depth to groundwater will be measured using an electronic water level indicator to determine groundwater elevations. During the sampling event, the locks and well caps will be inspected for damage or degradation and replaced as necessary. Groundwater samples will be collected from the wells in accordance with FDEP Standard Operating Procedures (SOPs) for Field Activities (DEP SOP-001/01). Prior to

Ms. Miranda McClure FDEPDC002 P13-82530

Florida Department of Environmental Protection March 28, 2019 collecting the groundwater sample, each well will be purged and field parameters including temperature, pH, specific conductance, dissolved oxygen, turbidity, and oxidation-reduction potential will be monitored. Purge volumes and field parameters will be recorded on field data sheets, and the field monitoring equipment will be calibrated in accordance with the manufacturer’s recommended procedures.

Groundwater samples will be collected using low flow sampling methods. The groundwater samples will be placed in laboratory-supplied containers, labeled with a unique identification number attached to the outside of the sample container, preserved on ice in a laboratory-provided cooler at a maximum temperature of four degrees Celsius, and delivered to a Florida Department of Health and National Environmental Laboratory Accreditation Program-certified fixed-base laboratory along with the completed chain-of-custody documentation to be analyzed for chlorinated

VOCs using EPA Method No. 8260B. For quality assurance purposes one trip blank will accompany the sample container for laboratory analysis of chlorinated VOCs using Environmental Protection Agency (EPA) Method 8260B.

Upon receipt of the laboratory analytical results, Golder will evaluate the groundwater concentrations to determine if off-site notification is required.

Golder will temporarily store purge water generated from the groundwater sampling event in a 55-gallon steel drum and treat the investigation derived waste (IDW) on site using Golder’s IDW treatment system. Based on the historical IDW characterization data, the treated aqueous IDW will be assumed to be below the regulatory standards.

Golder will spread the treated aqueous IDW on an impervious surface in locations sufficiently distant from any storm water drains to prevent runoff into the drains. The empty 55-gallon drum will be removed from the site following treatment.

SUBTASK 3 – REPORTING

The groundwater monitoring report will include a description of sampling procedures, evaluation of analytical results, comparison of current to historical data, tabulated summary of the current and historical analytical data, a site map showing the groundwater analytical results, water level elevation information in tables and figures, laboratory analytical data, conclusions, and recommendations. Golder will update the FDEP GIS tables with the new water level data, field parameter measurements, and VOC data; run the FDEP data checker on the completed tables; and submit the updated tables and error report under this subtask. Additionally, Golder will forward Automated Data

Processing Tool (ADaPT) data deliverables provided by the laboratory for the groundwater sampling event.

HEALTH AND SAFETY

Golder will update the existing site-specific Health and Safety Plan for the field work. The work will be completed in accordance with Golder’s safety policies and procedures. At a minimum, Golder will wear safety glasses, high visibility safety vests, and steel-toed boots. A safety job briefing will be conducted prior to the start of work activities, and anytime the scope of work or site conditions change.

COST AND SCHEDULE

The estimated cost to complete the monitoring is $3,520.75. Golder will tentatively schedule the sampling event for April 2019. The groundwater sampling event is estimated to require one day on site with two field personnel. The LOE/CEF cost sheets are included in Attachment A. The following table summarizes the proposed costs and deliverable schedule:
