HW561 DC003L
This Task Assignment authorizes Golder to conduct groundwater monitoring activities in accordance with the February 11, 2021, Scope of Work and Cost Estimate.
Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.
Subtask 2: Biennial Groundwater Monitoring and IDW Treatment Golder will collect groundwater samples from monitoring wells MW001I, MW001D, MW006I, MW007I, and MW012I – MW016I. Prior to sampling, depth-to-water will be measured. The wells will then be purged, and field parameters will be recorded. The groundwater samples will be collected using low flow sampling methods and analyzed for chlorinated volatile organic compounds (VOCs). Investigation-derived waste (IDW) will be temporarily stored in a 55-gallon drum before being treated with Golder’s aqueous IDW treatment system and disposed of on site.
Subtask 3: COVID-19 Costs Costs related to the use of extra vehicles so that Golder can maintain social distancing during the COVID-19 pandemic are covered under this subtask for invoicing purposes.
Subtask 4: Reporting Golder will submit the Biennial Groundwater Monitoring Report and ADaPT and GIS deliverables to FDEP.
The original Task Assignment totaled $7,286.10 with 53 labor hours but after negotiations and corrections with contractor costs are reflected as below:
The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $7,236.35 with 53 labor hours.

PROPOSAL FOR 2021 BIENNIAL GROUNDWATER MONITORING
FORMER COIN LAUNDRY EQUIPMENT OF JACKSONVILLE
1692 HAMMOND BOULEVARD
JACKSONVILLE, DUVAL COUNTY, FLORIDA
FACILITY IDENTIFICATION NUMBER: ERIC_4627
PROPOSE TASK ASSIGNMENT: DC003L
Dear Mr. Grubbs, Golder Associates Inc. (Golder) is pleased to provide the Florida Department of Environmental Protection (FDEP) with this cost proposal for annual groundwater monitoring at the former Coin Laundry Equipment of Jacksonville (Coin Laundry) site, located in Jacksonville, Duval County, Florida. This proposal includes a description of the proposed scope of services, level of effort (LOE) sheets for each subtask, and a Cost Estimate Form (CEF) that summarizes the costs of the proposed services. This scope of work is subdivided into three separate subtasks, as described below.
SUBTASK 1: PROJECT MANAGEMENT
To facilitate project quality and satisfactory completion of the proposed scope of work, the project will be overseen by an experienced project manager. The project manager will be responsible for the schedule, budget, invoicing, project activities, and communications with FDEP and the field team. Project management will include coordination of field activities; resource allocation such that the work is conducted by qualified professionals in accordance with the project schedule; cost control and tracking of the budget status; and submittal of project invoices in accordance with FDEP Drycleaning Solvent Cleanup Program requirements. The project manager will be responsible for the work being conducted in a manner that is consistent with the requirements of the FDEP-approved task assignment.
SUBTASK 2: ANNUAL GROUNDWATER SAMPLING AND IDW TREATMENT
Golder will collect groundwater samples from nine monitoring wells (MW001I, MW001D, MW006I, MW007I, MW012I, MW013I, MW014I, MW015I, and MW016I). Figures 2, 3, and 4 from the 2019 annual monitoring report, which show the potentiometric contours for the intermediate zone of the surficial aquifer and the volatile organic compound (VOC) results for the intermediate, and deep zones of the surficial aquifer, are attached.
Prior to groundwater sampling, depth-to-water measurements will be collected at the nine previously mentioned monitoring wells and existing monitoring wells MW002I, MW008I, and MW009I using an electronic water level
Mr. Benjamin Grubbs CX21455085
Florida Department of Environmental Protection February 11, 2021 indicator to determine groundwater depth. During the sampling event, the locks and well caps will be inspected for damage or degradation and repaired or replaced as necessary.
Groundwater samples will be collected from the wells in accordance with FDEP Standard Operating Procedures (SOPs) for Field Activities (DEP-SOP-001/01). Prior to collecting the groundwater sample, each well will be purged and field parameters including temperature, pH, specific conductance, dissolved oxygen, turbidity, and oxidation-reduction potential (ORP) will be monitored. Purge volumes and field parameters will be recorded on field data sheets, and the field monitoring equipment will be calibrated in accordance with the manufacturer’s recommended procedures.
Groundwater samples will be collected using low flow sampling methods. The groundwater samples will be placed in laboratory-supplied containers, labeled with a unique identification number attached to the outside of the sample container, preserved on ice in a laboratory-provided cooler at a maximum temperature of four degrees Celsius, and delivered to a Florida Department of Health and National Environmental Laboratory Accreditation Program certified fixed-base laboratory along with the completed chain-of-custody documentation to be analyzed for chlorinated VOCs using EPA Method 8260B. For quality assurance purposes, one trip blank will accompany each sample cooler for laboratory analysis of chlorinated VOCs using EPA Method 8260B. Upon receipt of the laboratory analytical results, Golder will evaluate the constituent concentrations to determine if off-site notification is required.
Golder will temporarily store purge water and development water generated from the monitoring well installation and groundwater sampling event in 55-gallon steel drums and treat the IDW on site using Golder’s IDW treatment system. Based on the historical IDW characterization data, the treated aqueous IDW will be assumed to be below the regulatory standards. Golder will spread the treated aqueous IDW on an impervious surface in locations sufficiently distant from any storm water drains to prevent runoff into the drains. The remaining 55-gallon drum(s) will be removed from the site following treatment.
SUBTASK 3: COVID-19 COSTS
Providing a second truck for use by a two-person field crew during a one-day field effort associated with Subtask 2 to provide protection from Covid-19.
SUBTASK 4: REPORTING
The biennial groundwater monitoring report will include a description of the sampling procedures, evaluation of the analytical results, comparison of current to historical data, tabulated summary of the current and historical analytical data, a site map showing the groundwater analytical results, water level elevation information in tables and figures, laboratory analytical data, conclusions, and recommendations. Golder will also update the FDEP GIS tables with the new water level data, field parameter measurements, and VOC data; run the FDEP data checker on the completed tables; and submit the updated tables and error reports under this subtask. Additionally, Golder will forward the Automated Data Processing Tool (ADaPT) data deliverables for the groundwater sampling event provided by the laboratory.
HEALTH AND SAFETY
Golder will update the existing site-specific Health and Safety Plan for the field work. The work will be completed in accordance with Golder’s safety policies and procedures. At a minimum, Golder will wear safety glasses, high visibility safety vests, and steel-toed boots. A safety job briefing will be conducted prior to the start of work activities, and anytime the scope of work or site conditions change.
Mr. Benjamin Grubbs CX21455085
Florida Department of Environmental Protection February 11, 2021
COST AND SCHEDULE
