Summary
This is a Task Assignment Notification Form and associated cost proposal documentation under Florida Department of Environmental Protection (FDEP) Consolidated Contract HW560. The contract involves three parties: the FDEP (state agency), Tetra Tech, Inc. (contractor), and Eurofins TestAmerica (analytical laboratory subcontractor). The task assignment addresses environmental assessment activities at the Fosgate Road Dump/Montverde Geophysics Site in Lake County, Florida, as part of the U.S. EPA Superfund State Political Subdivision and Indian Tribe Site Specific Cooperative Agreement.
The scope of work comprises three subtasks: (1) Project Management ($875.50), (2) Site Inspection Workplan/Quality Assurance Project Plan (QAPP) and Health and Safety Plan (HASP) ($1,520.07), and (3) Private Well Sampling including mobilization, sample collection from up to five residential wells, laboratory analysis for pesticides, and field documentation ($7,276.76). The total fixed-price task assignment value is $9,672.33 with no retainage. Work execution is scheduled through April 30, 2020, with deliverables due by April 20, 2020. The contract is on a fixed-price basis with invoicing at 100% completion upon approval of deliverables. Key assumptions include FDEP coordination of homeowner notification, FDEP coordination of sample analysis at the USEPA laboratory, and Tetra Tech responsibility for providing sample bottles, coolers, and shipping.

Subject: Revised Cost Proposal (Second Revision)– Fosgate Dump Private Well Sampling, Monteverde, Lake County, Florida
Dear Ms. Shirah:
Tetra Tech is pleased to submit to the Florida Department of Environmental Protection (FDEP) this cost proposal for private well sampling in the vicinity of the Fosgate Dump Site in Monteverde, Lake County, Florida. Services provided in this proposal will be completed as subtasks including: Subtask 1, Project Management; Subtask 2, Health and Safety Plan (HASP);
ubtask 3, Fieldwork including private well sampling and a deliverable package. These subtasks will include the following activities:
Subtask 1 - Project Management: Project Management includes managing the project scope, schedule, and budget; and project communication. The level of effort is based on the HW560 Contract requirements and assumes 5 hours for Task 1.
Subtask 2 – Work Plan and HASP: This task includes completion of a work plan and brief HASP (short form) for the residential well sampling event. The level of effort assumes 1 hour for a Senior Scientist to review the documents, 12 hours for a Scientist to prepare the workplan; 2 hours for a Staff Engineer to complete the HASP and coordinate review and signature by the corporate health and safety officer; 2 hours for graphics, and 1 hour for editing and document production.
Subtask 3 – Private Well Sampling: This subtask includes mobilization to and from the site area and visiting residences in the site area to request permission to sample private wells, collecting private well samples for pesticides, shipment of the samples to the USEPA laboratory, and field documentation. These activities will be conducted by a Senior Scientist and Staff Scientist. The level of effort, equipment, and supplies needed for this subtask are described in the cost estimate
Tel 850.385.9899 Fax 850.385.9860 www.tetratech.com worksheets. Private wells to sample have not yet been identified, but the objective is to collect samples from up to five private wells, with a minimum expectation of sampling three wells.
The field personnel will mobilize from the Tetra Tech Tallahassee office. Total hours for the Senior Scientist and the Staff Scientist, including equipment staging, field work, mobilization, and field deliverable are 22 and 24 hours, respectively. The field operations staff in Tetra Tech’s Tallahassee office assigned to this project have extensive experience with the FDEP CERCLA Site Screening program and working specifically with the FDEP Team. They have an in depth understand of the program’s needs and standard operating procedures for site inspections. All staff are thoroughly trained and experienced in the USEPA CERCLA program’s standard field procedures.
The field work is anticipated to be completed in two days, including mobilization and demobilization, however the workday schedule and length of time necessary to get permission and pull samples is dependent on homeowners granting access and being available to allow entry to sample their wells.
The field operations lead will be in communication with the FDEP project manager during field activities to provide updates on progress and schedule Tetra Tech will obtain approval from FDEP in advance if additional time would be needed to complete the work. The deliverable for this subtask will be a documentation package containing field notes, copies of chain of custody and shipping air bills, GPS coordinates of wells sampled and photographs of the wells. This will be provided to FDEP within two weeks of field work completion.
Tetra Tech’s cost proposal is based on the following assumptions:
FDEP will send letters to homeowners in the targeted area requesting permission to sample their wells and notifying homeowners of Tetra Tech’s role in the project and field schedule. FDEP will provide Tetra Tech with copies of the letters and a map showing residences contacted prior to mobilization.
FDEP will coordinate sample analysis and shipment to the USEPA laboratory, but Tetra Tech will provide sample bottles and coolers. Tetra Tech will not be responsible for entry or submittal of sample information into the USEPA SCRIBE program. Tetra Tech will be responsible for shipping samples to the location provided by FDEP.
The field activities will be conducted during the week of April 6, 2020 to meet the laboratory schedule.
Investigation derived waste (IDW) will be minimal and will not be containerized.
Tetra Tech is prepared to conduct work on this project following receipt of a signed FDEP task order. A signed task order will reflect the approval of this proposal and authorize use of associated funding required to commence project activities. The total fixed price cost to complete the activities described above is $9,672.33.
Please find a cost summary attached that provides details concerning this cost proposal. Tetra Tech anticipates completion of this project on a fixed price basis and will invoice costs incurred at 100% completion based on approval of deliverables and in accordance with deliverable due dates.
Period of Performance Tetra Tech will begin work on the work plan and HASP upon receipt of the signed task assignment and will be prepared to mobilize to conduct the work on the week of April 6, 2020. The field documentation package will be provided promptly after field work has been completed.
If you have any questions, please call myself or Richard Hicks (850) 385-9899.

Negotiation Summary – Task Assignment (TA) CSS-26A Fosgate Road Dump, COM_367006, Montverde, Lake County Site Inspection (SI) Supplemental March 13, 2020
The cost associated with the revised Cost Proposal submitted on February 22, 2020 was $7,885.43. Negotiations between FDEP and Tetra Tech, Inc. concluded with the following modifications:
Additional deliverable (Site Inspection Workplan/QAPP) was needed for Subtask 2 to meet EPA requirements.
Sample bottles and coolers are to be obtained by Tetra Tech, Inc. (instead of FDEP providing them).
Due to these changes, the agreed upon final cost of this is $9,672.33.
