About this file
This document is a Task Assignment Notification Form for the Florida Department of Environmental Protection (FDEP) under Contract HW559. The assignment authorizes Wood Environment & Infrastructure Solutions, Inc. to conduct annual groundwater monitoring activities at the Harveys International Dry Cleaners site (ERIC_4609) located at 6348 103rd Street, Jacksonville, Duval County, Florida. The contract is managed by FDEP Contract Manager Kevin Kane, with Geoff Schaefer serving as the contractor representative. The task assignment is classified as a Fixed Price arrangement with a total not-to-exceed value of $5,081.98 and zero retainage.
The scope of work consists of two subtasks: Project Management (calculated at 12.5 percent of direct labor) and Groundwater Monitoring. Under Subtask 2, Wood will conduct groundwater sampling at four micro-wells (MW-1S, MW-2S, MW-1I, and MW-8I), measure water levels at all ten micro-wells on site, and analyze samples for chlorinated volatile organic compounds (CVOCs) using USEPA Method 8260 by Advanced Environmental Laboratories. The contractor will also manage the disposal of investigation-derived waste (IDW), with Capitol Environmental Services providing non-hazardous IDW drum transport and disposal at $145.00 per 55-gallon drum. The final deliverable—an Annual Groundwater Sampling Report and ADaPT/GIS update submitted electronically—is due October 31, 2021, with task completion scheduled for February 25, 2022. Payment of the full $5,081.98 is conditioned upon FDEP approval of the Annual Groundwater Sampling Report and completion of both subtasks.

HW559 DCOO3i Task Assignment
DATE: June 29, 2021
This Task Assignment authorizes Wood Environment & Infrastructure Solutions, Inc. to conduct annual groundwater monitoring activities, as outlined in the May 5, 2021 (Rev.
June 23, 2021) Cost Proposal to Conduct Annual Groundwater Sampling Harvey’s International Drycleaners (former). Specific subtasks are detailed below.
Subtask 1: Project Management Project management is calculated at 12.5 percent of the direct labor.
Subtask 2: Groundwater Monitoring Wood Environment & Infrastructure Solutions, Inc. will conduct groundwater sampling at micro-wells MW-1S, MW-2S, MW-1I and MW-8I. Additionally, water levels at all 10 micro-wells present on site will be measured. IDW disposal is expected and has been accounted for under this subtask. Wood was unable to obtain a third IDW disposal quote due to a lack of response from Perma-Fix Environmental Services, Inc. Samples will be analyzed for chlorinated volatile organic compounds (CVOCs) using USEPA Method 8260 by Advanced Environmental Laboratories in Tallahassee, Florida. Upon receipt of the data, the contractor will generate a Groundwater Monitoring Report detailing the field activities, procedures, and analytical results.
Wood’s time, labor and subcontractor costs for these activities have been reviewed and are appropriate.
The level-of-effort and final cost of $5,081.98 is reasonable for the scope of services.
Task completion date is February 25, 2022.

RE: Cost Proposal to Conduct Annual Groundwater Sampling
Harvey’s International Drycleaners (former) 6348 103rd Street Jacksonville, Duval County, Florida
ERIC_4609
FDEP FAC ID No: 16/9700173
Dear Mr. Meyers, Wood Environment & Infrastructure Solutions, Inc., (Wood) has prepared this cost estimate to continue annual groundwater monitoring at the former Harvey’s International Drycleaner site in Jacksonville, Florida. All sampling activities conducted during this groundwater sampling event will be in accordance with the Standard Operating Procedures (SOPs) described in the original Site Assessment Workplan dated November 6, 2014. The last sampling event at the site took place on May 28, 2021. Figure 3 shows the locations of the existing micro-wells to be sampled.
The proposed scope of work is detailed below:
Annual Groundwater Sampling Event
Wood will mobilize a two-person crew to the site to conduct the groundwater sampling activities.
Prior to collecting groundwater samples, all 10 micro-wells will be opened and allowed to equilibrate and then water level measurements will be collected. Groundwater samples will then be collected from four micro-wells including MW-1S, MW-2S, MW-1I and MW-8I and analyzed for chlorinated volatile organic compounds (CVOCs) using USEPA Method 8260 by Advanced Environmental Laboratories in Tallahassee, Florida. Samples will be shipped overnight under standard chain of custody protocol.
All purge water will be containerized in a 55-gallon drum. At the end of the event, an IDW sample will be collected from the drummed purge water and submitted for laboratory analysis. The drum will be staged at a secure location onsite pending the analytical results. If the analytical results indicate that CVOCs are all below their respective groundwater cleanup target levels (GCTLs), the purge water will be emptied onto an impervious surface. If CVOCs are detected above their respective GCTLs, the drum will need to be transported from the site for disposal since groundwater sampling is occurring on an annual basis.
As a deliverable for this event, Wood will prepare an Annual Groundwater Monitoring Report detailing the activities and findings from the annual sampling event including updated tables and figures. The figures will include the current contaminant plume based on the most recent analytical data, as well as a figure depicting the groundwater flow at the site.
The total estimated cost for the proposed activities is $5,081.98. The sampling event will be conducted within 30 days of receiving a signed Task Assignment.

Based on the results of the sampling events, Wood will make recommendations for future work that may be necessary at the site. Please let me know if there are any questions concerning this proposal.

Hey Dale
Can you send me a quote with rates for disposal that I can use for drycleaning sites for like the next 6 months? Maybe something with a cost for up to _X_ drums and then a quote per drum for multiple drums over _X_ drums. Please consider all to be non-hazardous.
Please let me know if you have any questions. These sites will be located in North Florida, mainly Jacksonville mailto:geoff.schaefer@woodplc.com mailto:Kevin.Kane@FloridaDEP.gov file:////c/www.woodplc.com mailto:mdale@perma-fix.com mailto:ssierens@perma-fix.net
Thanks Ron
