SUBJECT: Negotiation Summary- Task Assignment Change Order #1 (BF07E-1)

Gilchrist County/Former Colt’s Station Supplemental Site Assessment Activities/Tank Closure Report Facility ID # 21/8508484, ERIC_5564

This Task Assignment Change Order (TACO) is intended for conducting additional assessment of the former aboveground storage tank (AST) area where a previous spill and a small source removal occurred. Preliminary screening results from a temporary monitor well and two soil borings in the former AST area (Subtask 2) indicate groundwater contamination at depth. This TACO also adjusts reduced analytical costs and extends the dates for the interim deliverables and the final deliverable.

The purpose of these supplemental site assessment activities (Subtask 4) is to provide the required information to the DEP Northeast District in order to achieve an NFA for the Supplemental Site Assessment and Tank Closure, and for the issuance of a Site Rehabilitation Completion Order (SRCO) under one of the Risk Management Options as described in Rule 62-780.680, Florida Administrative Code (F.A.C). This work is being conducted using grant funds from the State & Tribal Response Program (SRP) Grant that is administered by DEP on behalf of EPA.

Subtask 1: Project Management This subtask is a percentage of the overall project and has been negotiated as a contract rate. It includes project management hours, direct labor and overhead costs. There is a total addition of $613.97 to Subtask 1. The cost of this subtask is $1,231.52.

Subtask 2: Supplemental Site Assessment Activities Under this subtask, Wood E&IS is removing staff member Phil Bambach and adding Nick Stoudt due to Mr. Bambach recently leaving the company. This subtask also covers additional costs totaling $130 for the DPT subcontractor for materials related to the two installed piezometers. Also covered under Subtask 2 is a <$1,866.50> reduction in costs due to all of the groundwater samples not being able to be collected due to dry-hole conditions. These changes result in an overall decrease of <$2,054.13>. The resultant total of this subtask is $7,763.05.

Subtask 3: Supplemental Site Assessment/Tank Closure Report Upon completion of the field activities and receipt of the analytical data, Wood E&IS will prepare a Supplemental Site Assessment/Tank Closure Assessment Report detailing the findings of both the Subtask 2 and 4 contamination assessment activities including tables and figures. At a minimum, tables will include soil and groundwater laboratory data and a field table showing lithology, OVA results, and sample locations; figures will include a site location map, detailed sample location map, contamination plume maps, and direction of groundwater flow. This TACO will cover the removal of

Negotiation Summary 07/31/2018 hours for Mr. Bombach (hourly rate of $23.29) and the addition of hours for Alex Price (hourly rate of $22.56). Four additional hours (2 hours each for Ron White and Alex Price) have been added to allow time for additional sample tabulation and report preparation. These changes represent a total increase of $384.08 for this subtask. The overall cost of this subtask is $2,992.59.

Subtask 4: Supplemental Site Assessment Activities Under this subtask, Wood E&IS will further define the horizonal and vertical extent of groundwater contamination in the former AST area. Wood E&IS will install three, 2-inch diameter (ID) permanent monitor wells in and around the former AST area. These monitor wells will have a 10-foot screen and be set to an approximate total depth of 45 feet. The three monitor wells will be sampled for VOAs (BTEX/MTBE) using USEPA Method 8260, PAHs using Method 8270, TRPHs using the FL-PRO method, ethylene dibromide (EDB) using EPA Method 8260 and lead using EPA Method 6010 and sent to Advanced Environmental Laboratories (AEL) of Tallahassee, Florida for laboratory analysis. In addition, water levels with be collected from the newly installed monitor wells and the top-of-casing elevations for wells will be surveyed in to determine the groundwater flow direction. The cost of this subtask is $15,644.55.

Summary DEP’s SRP Contract Manager and Wood E&IS’s Contract Manager have both agreed to the scope of work and associated costs. The final cost associated with this TA and including the CO is $27,631.72.

Documentation of these revised costs is included in the TACO Level of Effort (Delta and Final)/Cost Estimate Form and cover letter provided by Wood on July 27, 2018, as well as the Task Assignment Form for BF07E prepared by DEP. Final invoicing must be completed no later than 9/30/18.

RE: Change Order #1 (TA# BF07E) to Conduct Supplemental Site Assessment

Former Colt’s Station Site Trenton, Gilchrist County, Florida FDEP FAC ID No: 21/8508484

Dear Jim, Wood Environment & Infrastructure Solutions, Inc., has prepared this change order per your request to complete the supplemental site assessment activities that were conducted on July 11 & 18, 2018 at the Colt’s Station site in Trenton, Florida. Those SSA activities included additional assessment at the former AST area where a previous spill occurred and small source removal was conducted. The SSA activities were conducted on July 11, 2018 and included advancing soil borings to the water table and collecting soil samples. However, due to difficult lithologic conditions, the proposed groundwater samples could not be collected through the DPT rig screen point sampling system. It was decided to install two temporary piezometers (TW-1 & TW-2) with 10-foot screen intervals were set to a depth of approximately 40 feet below land surface (bls), however, the piezometers did not contain any groundwater. Saturated soils were observed starting at approximately 26 feet bls in the soil borings advanced at these locations. It was decided to return to the site the following week after groundwater had ample time to accumulate in the piezometers. Wood returned to the site on July 18, 2018 to collect the groundwater samples. After a full week, less than 1-foot of water had accumulated in the piezometers. Per FDEPs instruction, TW-2, which was installed in the former UST pit, was not sampled and the temporary piezometer was abandoned in place with grout. Wood was able to collect a groundwater sample from TW-1 with the use of a peristaltic pump with a check valve on the end of the tubing. Although the temporary well had less than 1-foot of water in it, Wood was able to collect all of the sample parameters, however, the turbidity was very high. A filtered sample was also collected for the lead analysis. After TW-1 was sampled, it was also abandoned in place by grouting the well to the surface. These sample results have not yet been received from the laboratory. FDEP has requested this change order to return to the site to install three, 2-inch diameter monitoring wells with hollow stem augers using a conventional drill rig. Wood has prepared this change order to include costs for this additional work plus addresses some changes in Subtask 2 of the TA.

The changes are described below:

Subtask 2 – Supplemental Site Assessment Activities The changes under this task include changing Wood personnel from Phil Bambach to Nick Stoudt because Mr. Bambach has recently left the company. This labor category change resulted in a decrease of $295.30 for labor charges.

This change order also includes additional costs for the DPT subcontractor for the material for the installation of the two temporary piezometers. These changes were approved by Jim McCarthy in the field. This change results in an increase of $130.00 for Preferred Drilling. This change order also includes a reduction in costs for Advanced Environmental Laboratories of $1,866.50 because all of the groundwater samples were not collected due to lack of water.

These changes result in an overall decrease of $2,054.13 for Subtask 2.

Subtask 3 – SSA Report This change includes removing hours for Mr. Bambach and adding those hours for Ms. Price plus adds 4 hours of additional labor (2 hours each for Ron White & Alex Price) to allow more time for additional sample tabulation and report preparation. This change results in an increase of $384.08.

Subtask 4 – SSA Activities Per FDEPs request, Wood will oversee the installation of three permanent monitoring wells at the site. The locations of the three monitoring wells are shown on the attached figure. Each well will be installed through HSAs using a conventional drill rig and will be installed to a depth of 46 feet bls. This depth was agreed upon based on monitoring wells on a nearby petroleum site. Each well will be constructed on 2-inch diameter, Schedule 40 PVC with a 10-foot, 0.010-inch slotted screen interval and will be completed within a steel manhole embedded in a flush-mounted, traffic-bearing, 2’x 2’ concrete pad. Since the soil screening OVA data collected on July 11th indicated that the soil in the vicinity of the former AST pit are contaminated, the drill cuttings generated from each well will be containerized in 55-gallon drums for disposal. The transportation and disposal of wastes will be handled under an additional TACO since the exact number of drums necessary is not known. After completion, each monitoring well will be developed until it is sediment-free to the approval of Wood personnel. Each monitoring well top of casing elevation will be surveyed to an arbitrary elevation and depth to groundwater measurements will be collected to help determine the groundwater flow direction at the site.

The following week, Wood will return to the site to collect groundwater samples from the three newly installed wells. The groundwater samples will be collected with a submersible pump and new HDPE tubing. The groundwater samples will be analyzed for VOCs and ethylene dibromide (EDB) using EPA Method 8260, PAHs using USEPA Method 8270D, TRPHs using the FL-PRO method and lead using EPA Method 6010. These changes result in an increase of $15,644.55 for Subtask 4.

All sampling activities conducted during this supplemental site assessment will be in accordance to the FDEP Standard Operating Procedures (SOPs) described in the Tank Closure Workplan dated March 2015.

Upon completion of the field activities and receipt of the analytical data, Wood will prepare a Supplemental Site Assessment Report detailing the findings of these SSA activities including updated tables and figures.
